We are looking for a friendly, organized, and detail-oriented Patient Intake Manager to join the Bestcare team. In this role, you will be one of the first points of contact for new referrals, clients, family members, and contracted partners. You will help guide referrals through the intake process, coordinate with internal teams, and ensure new cases move smoothly from referral to authorization and scheduling. What You’ll Do · Receive, review, and respond to new home care referrals received through HHA Exchange, phone, and email. · Evaluate referral details, including service area, aide availability, client needs, and language preferences. · Verify client eligibility or required insurance/payment information as needed. · Communicate with prospective clients, family members, representatives, referral sources, and contracted partners in a professional and helpful manner. · Coordinate accepted cases with branch leadership and internal teams for assignment and next steps. · Process referrals for home care services, including MLTC, Hospice, CHHA, and private pay when applicable. · Track referrals, follow up on authorizations, and help ensure timely case starts. · Maintain accurate intake records, referral documentation, and communication notes.
