Job Description JWCH Institute, Inc. is a non-profit Federally Qualified Health Center (FQHC) with a mission to provide comprehensive health care to low-income and homeless people in the County of Los Angeles. For many people, gaining access to dental care isn’t simply a matter of finding the nearest dentist. Some may face language and cultural issues that keep them from connecting with dental providers. Others may lack insurance coverage, transportation or childcare that would make it easier for them to visit the dentist. JWCH works to assist patients with navigating barriers and helping them achieve better oral health and improved overall health outcomes. Position Purpose: This position is responsible for assisting dentists in the direct provision of primary dental care services to patients at the clinic. The Registered Dental Assistant (RDA) is also responsible for the sterilization, preparation, and inventory control of dental instruments and supplies, as well as ensuring proper infection control in all patient care areas. RDAs are vital members of the JWCH Dental Department and must contribute to fostering a team atmosphere, whether at their assigned clinic or when filling in at other sites. RDAs are expected to use AIDET as an internal and external customer service tool with both patients and JWCH staff. Job Description JWCH Institute, Inc. is a non-profit Federally Qualified Health Center (FQHC) with a mission to provide comprehensive health care to low-income and homeless people in the County of Los Angeles. For many people, gaining access to dental care isn’t simply a matter of finding the nearest dentist. Some may face language and cultural issues that keep them from connecting with dental providers. Others may lack insurance coverage, transportation or childcare that would make it easier for them to visit the dentist. JWCH works to assist patients with navigating barriers and helping them achieve better oral health and improved overall health outcomes. Position Purpose: This position is responsible for assisting dentists in the direct provision of primary dental care services to patients at the clinic. Additionally, the Dental Assistant is responsible for sterilizing, preparing, and maintaining inventory control of dental instruments and supplies, as well as ensuring proper infection control in all patient care areas. The Transportation Coordinator is responsible for transporting clients to and from appointments and exit destinations. Sending out daily schedule for the appointments entered in the calendar. Maintaining the maintenance on vehicles as needed. The pay rate for this position is: $22 per hour. ABOUT THE JOB As part of the Interim Housing team, the Lead Case Manager will assist clients in breaking the cycle of homelessness by supporting clients in various phases, from street to housing. The lead case manager is a seasoned social services professional who collaborates with the multidisciplinary team and the team/program supervisor to support the needs of the client. The lead case manager maintains a direct client caseload and takes on additional duties to support the operations of the program and the program manager. This may include serving as a point of contact for funders, providing support and training to case management staff, and overseeing specific administrative functions of the program Position Responsibilities include: • In collaboration with the Quality Assurance Department, ensure programs are in compliance with contracts and partnership agreements., • Under the supervision of the program manager, function as a contact representative for program matters., • Under the supervision of the program manager, provide administrative oversight of the team, which may include coverage for the program manager., • Support with the completion of critical incident reports as required per PATH policies and/or the contract., • Conducts intake and individualized needs assessment for all clients and collaborates to develop Individualized Service Plan (ISP) that addresses housing goals, maintaining/increasing income and other personal goals identified by the client including medical, mental health, substance use, financial resources, vocational, and social support needs., • Provides referrals, linkages, information, and support to resources that help clients to achieve their ISP goals., • Completes program specific assessments (e.g. VI-SPDAT)., • Evaluates strengths and challenges to addressing short term and long-term goals, conducts a 90- day review with the client after initial intake., • Maintains documentation standards as required by the Program and PATH policies, including completing case file notes in a timely manner and maintains a complete, accurate, and current client file with all required documents., • Develops therapeutic and/or professional relationships with clients and systems/partners that enhance efforts to engage and retain individuals with their housing goals., • Provides crisis intervention and risk assessment services focused on increasing safety and enhancing the clients' ability to independently solve problems, utilize effective coping skills, and manage and self -coordinate own care, including services for individuals with chronic trauma., • Uses motivational interviewing and trauma informed care in service delivery., • Maintains current, thorough knowledge of community resources and utilizes them to provide comprehensive, wrap-around services to clients. Essential Functions • Meet with each participant at least once a week or as needed to: assess, support, motivate and encourage participant to work towards their goals and/or needs and provide ongoing support by practicing a “whatever it takes” approach and Trauma Informed care, Harm Reduction and Housing First Models., • Complete initial psychosocial/mental health assessment; develop an individual service plan (ISP) and monthly action plan with each assigned participant, to establish and track client goals, including action steps and timeframe for completion of each step leading to securing permanent housing., • Strengthen job readiness skills through coaching in job search, applications, resume writing, interviewing and follow-up skills., • Assist each client in developing a personal budget, including establishing or increasing their income and/or creating a savings plan. If applicable to program, maintain accurate records of client income and/or savings., • Complete the LA HAT Assessment, the revised Coordinated Entry System (CES), with all eligible clients to ensure they are connected with the CES system and collaborate with SSVF and HUD VASH to link Veterans to potential housing resources., • Regularly and accurately maintain client data in HMIS/Clarity and paper file, including complete documentation/case notes of all activities, services, and outcomes achieved. Update case notes and general client information within 72 hours of meeting with client., • Responsible for submitting all required program reports and/or documents in a timely manner., • Collaborate, Develop and foster linkages with other social service agencies to provide clients with access to a broad range of supportive services, including but not limited to: VA Liaisons, VASH, SSFV, HVRP, CBEST, legal services, transportation services, health/mental health services, employment preparation/placement services, substance abuse services, vocational services, access to mainstream benefits, and identification of and access to permanent housing., • Responsible for deescalating, investigating, and/or problem-solving participant incident reports and/or grievances. Position Summary The Program Case Manager at the Santa Fe Springs Transitional Living Center provides direct supervision and case management services for residents at our sites. The Whittier Navigation Center is a program for 139 homeless men, women, and couples who are Whittier residents. TLC is a transitional housing and supportive services program for 28 homeless women and their children, and 12 older adult women at our West Wing area. Time Limited Subsidy is a program for individuals who need assistance locating and sustaining independent housing. The functions for this position include Case Management Services: Intake, assessment, individual service planning, ongoing monitoring, and follow-up activities, enforcing the program rules and regulations as well as providing support and encouragement to the clients. Essential Functions • Maintain documentation in accordance with all regulatory and internal policies respectively for each housing program’s contract requirements, including: weekly progress/case notes, data entry, paperwork, and reports., • Complete a Coordinated Entry System (CES) Triage Tool and enter the information into Homeless Management Information System (HMIS) and also keep highly organized files for each client and enter appropriate data into the region’s Homeless Management Information System (HMIS)., • Participate in regular case conferencing calls with the Department of Mental Health (DMH) Interim Housing Program (IHP). Prepare progress notes and complete all data entry requirements, necessary paperwork and documentation for contract requirements., • Collaborate with clients’ mental health workers from DMH and contracted agencies to support mental health engagement and stability., • Conduct Case Management Services to include: Intake, assessment, individual service planning, ongoing monitoring and follow-up activities., • Facilitate initial orientation for new residents including intake, room inventory, if needed, child enrollment in local school and after school program, and other local service referrals., • Meet regularly with clients to develop short and long-term goals, outcomes management, monitoring savings and if needed, reassess needs and progress., • Assist clients with implementing a housing plan based on client preferences and taking into consideration the available housing resources that are in alignment with the CES Triage Tool score., • Develop effective, trusting relationships with participants, with a focus on facilitating independence and maintenance of improved physical and mental health., • Assist clients with obtaining identification, Birth Certificates, and necessary documents for expediting access to health, mental health, substance use treatment, housing and other services., • Provide linkages to transportation resources including: bus, taxi, van, Access Services, and/or other possible options to ensure participants’ access to supportive services., • Must drive to participant location to meet case management services., • Perform room checks respectively for each client and notify management regarding maintenance issues., • Build and maintain a documented network of referrals and resources for clients, and foster relationships with local service providers and agencies to increase effective communication and referrals., • Support client discharge process to transitional or permanent housing: provide counseling during the transition, refer clients to relevant resources that support long term housing and income stability, and complete outcome reports within 24 hours upon exiting., • Facilitate House Meetings to empower clients and address any program concerns that arise., • Mediate between clients if conflicts arise and issue program violation warnings if needed., • Maintain an ‘on-call’ status and be available to respond to the facility on an emergency basis., • Collaborate with Program Director to ensure adherence to safety and security measures., • Attend program management meetings, Case plan support team meetings, staff meetings and trainings when, • Acts according to all emergency policies., • Support the goals and directives of the Program Team., • Follows through with all legal obligations (e.g. Child abuse reporting, Elderly abuse, accident reports, etc.) for government agencies as well as Salvation Army policies., • Work collaboratively with the facility monitors to ensure client/program compliance., • Other duties as requested or assigned by the Program Director or Designee.
