Job Description Job Summary We are seeking a compassionate and experienced Wound Care Nurse Practitioner (NP) to join our multidisciplinary healthcare team. The Wound Care NP will be responsible for providing comprehensive care to patients with acute and chronic wounds, including pressure injuries, diabetic foot ulcers, venous and arterial ulcers, and surgical wounds. This role includes advanced assessment, diagnosis, procedural treatment, and follow-up care, while working collaboratively with physicians, nurses, and other clinical staff to ensure optimal healing outcomes. Key Responsibilities · Conduct comprehensive assessments of wounds and overall skin integrity. · Develop and implement individualized treatment plans for wound management, including debridement, dressing selection, infection control, compression, and offloading strategies. · Perform wound care procedures such as selective, excisional, and enzymatic debridement; wound cultures; and application of advanced wound care products. · Evaluate for and initiate advanced modalities such as negative pressure wound therapy (NPWT), compression therapy, cellular and tissue-based products (skin substitutes), and hyperbaric oxygen referrals. · Order and interpret diagnostic studies (ABI/TBI, wound cultures, imaging, vascular studies, labs) to guide treatment plans and escalate care when needed. · Monitor patient progress and adjust care plans as necessary to promote optimal healing and prevent recurrence. · Collaborate with primary care providers, vascular surgery, podiatry, infectious disease, endocrinology, nutrition, and other specialists to ensure interdisciplinary continuity of care. · Communicate with home health, skilled nursing, and rehabilitation facilities to coordinate follow-up and supply management. · Educate patients and caregivers on wound care techniques, prevention strategies, and overall skin health. · Document all patient encounters, procedures, and progress using the electronic medical record (EMR) system. · Ensure documentation meets CMS and payer requirements for medical necessity and supports appropriate CPT and ICD-10 coding for E/M services, debridement, and advanced therapies. · Participate in outcome tracking, case reviews, and quality improvement initiatives to enhance clinical effectiveness and patient satisfaction. · Maintain compliance with facility protocols, regulatory standards, and infection prevention guidelines. Qualifications Required: · Licensed NP in NY, able to provide services under NY practice protocols; if under 3,600 hours, will engage in collaborative agreement with physician. · Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) · Current Nurse Practitioner license in New York · Board certification (FNP, AGACNP, or related specialty) · Certification in wound care (e.g., WCC, CWCN, or CWS) or willingness to obtain within 3 months of hire · BLS certification Preferred: · 2+ years of experience in wound care, dermatology, or a surgical subspecialty · Familiarity with advanced wound care technologies (NPWT, skin substitutes, compression systems) · Experience in outpatient, post-acute, or long-term care settings · Proficiency with EMR wound modules, digital planimetry, and photographic documentation Skills & Competencies · Strong clinical assessment and procedural skills · Excellent communication and patient education abilities · Ability to work independently and within a multidisciplinary team · Proficient in EMR systems and clinical documentation · Commitment to evidence-based practice, continuous learning, and quality outcomes Work Environment · May require travel between facilities or home health locations (if applicable) · Exposure to bodily fluids and infection risks; PPE provided · Physical demands include standing, walking, and lifting (up to 30 lbs) Salary & Benefits Salary Range: Commensurate with experience, certification, and location · Generous paid time off (PTO) and holidays · Continuing education reimbursement · Malpractice insurance coverage · Licensure and certification reimbursement · Flexible scheduling options (depending on setting) Job Types: Full-time, Part-time Work Location: In person
