POSITION OVERVIEW The Program Administrator of Substance Use Disorder Services is responsible for the administrative oversight of the OASAS certified 822 Comprehensive Integrated Outpatient Treatment Program. The program serves adults who have a primary substance use disorder diagnosis and those diagnosed with co-occurring substance use and mental health disorders. In collaboration with the Senior Administrator of Outpatient and MAT programs, the Program Administrator will ensure that her/his programs are accessible, effective, and compliant with all applicable regulations, guidelines, policies, and procedures and financially sustainable. As part of the management team, the Program Administrator will actively participate in the integration of behavioral healthcare services including the development and sustainability of a comprehensive, patient centered, trauma informed recovery model. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. SIJC is seeking a Clinician, Supervised Release Program (SRP). Reporting to the Clinical Director, the Clinician, SRP will be licensed a social worker and mental health practitioner responsible for providing therapeutically-informed support to participants. This position will leverage exemplary clinical and interpersonal skills to provide clinical support to participants enrolled in the program. Specifically, the Clinician, SRP will facilitate CBT-informed sessions for all Tier 2 participants, provide short-term counseling for any participant identified as a strong candidate for mental health support, and external referrals to therapy, as appropriate. This position provides hours towards LCSW accreditation. The Clinician, SRP will not carry a standard SRP caseload. Rather, they will work closely with the case management and specialty in-house referral teams to case conference about participants' strengths and needs and coordinate tailored support as appropriate. The Clinician, SRP will work with other SRP teams to ensure access to, completion of, and accurate data entry for CBT and other therapeutically-informed sessions to ensure these sessions count toward participants’ compliance status. Job Description JOB SUMMARY Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0CV-4165 (NGG) O’Toole et al. v. Cuomo et al., Civil Action No. 13-CV-4166 (NGG). This settlement requires the state to assist residents of impacted adult homes identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap-around services are in place after the move. An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. They will be responsible for making recommendations about the type of housing and the types of services—physical and behavioral health—as well as non-medical services like Meals on Wheels, etc. The Social Work Assessor will complete assessments for Queens Adult Home and SIBN Adult Home Supported Housing Programs. The Social Work Assessor will complete the housing application's psychiatric evaluation and psychosocial portion. The Nurse Assessor will have primary responsibility for assessing the residents' physical health needs, such as Managed Long-term Care services and certified Home Health Agency services. Each assessor will interface with the respective behavioral or physical health providers of the residents and review records necessary to make determinations about the types of services that will be needed to promote successful placement in supported apartments.
