The Opportunity Join One Medical's innovative High Risk team as a Telehealth Patient Support Specialist (internally known as Field Care Coordinator), where you'll make a meaningful difference in patients' lives by delivering personalized care beyond traditional clinic walls. In this role, you'll bring healthcare directly to patients' homes through state-of-the-art telehealth services, creating a bridge between patients and their healthcare providers. Reporting to the Tele-Assist Program Operations Manager, Field Care Coordinators go into patients’ homes to facilitate telehealth visits with a provider in order to reach more of our patients where they live. This will include building a trusted and supportive relationship with the patient and their family, performing clinical assessments, assessing home safety, documenting medications and reviewing provider care plans with the patient. You will also assist in teaching patients and family members to use technology to coordinate care with their care teams, as needed. The Opportunity One Medical's High Risk team is looking for a Community Health Worker (CHW) to act as the bridge between our patients, community resources, and healthcare services in order to remove barriers and increase wellness. Reporting to the Resource Navigation Program Manager, a CHW is a patient's advocate or liaison, accompanying patients through proactive in-person and virtual or telephone outreach to promote health literacy and increase access to resources needed to live healthier lives. As a key member of the High Risk Care team, CHWs work with patients to identify personal health goals, support care team decision making, and coordinate clinical and complementary services needed to provide high quality health care, while also improving the quality and cultural competence of service delivery. Organizational Overview The King County Regional Homelessness Authority (KCRHA) is a public administrative agency created by an interlocal agreement between the City of Seattle and King County. KCRHA is responsible for unifying, coordinating, and administering the funding of emergency services for people experiencing homelessness to provide oversight and accountability. Our mission is to administer effective, performance-based homeless services to support a high-functioning homelessness crisis response system to significantly decrease the incidence of unsheltered homelessness across King County using equity and social justice principles.
