    SCOPE OF ROLE: The Case Manager provides and coordinates client services. II. ESSENTIAL DUTIES & RESPONSIBILITIES: • Conduct intake process with new clients within 24 hours of admission; includes but is not limited to explanation of resident rights and responsibilities, and grievance procedures • Conduct psychosocial, vocational/educational, and housing assessments and documentation of same • Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing • Prepare housing applications and assist clients with the housing application/interview process., • Monitor and assess client progression/regression in behavior and/or appearance, • Facilitate group and individual counseling, workshops, psych education, and recreational activities, • Conduct room/dorm visits/inspections to assess client personal hygiene, room cleanliness, and overall, ADL skills • Provide comprehensive case management services to an assigned caseload, • Develop initial service plans and conduct service plan reviews as needed for an assigned caseload, • Provide referrals for in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate • Ensure that clients attend meals and maintain appropriate nutritional intake, • Provide crisis intervention and address all emergency situations as appropriate, • Assist in building evacuation during fire drills and building emergencies, • Monitor client medication compliance and collaborate with nursing staff monitor compliance, • Refer clients to appropriate community resources as needed, • Maintain CPR/First Aid Certification, • Maintain up to date, accurate, complete, and timely client chart documentation in compliance with DHS, OTDA, other Funder and SUS standards • Facilitate discharge/transfer process and documentation of same JOB DESCRIPTION • Attend staff meetings, case conferences, group supervision, training, and escort clients on housing interviews as needed • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes • A work schedule that includes some weekend and evening hours is required, • Other duties as assigned III. REQUIREMENTS REQUIRED EDUCATION AND EXPERIENCE • Bachelor’s Degree is Human Services field preferred, HS diploma or GED required, • Three years’ experience with homeless population, • Ability to work with a culturally diverse population PREFERRED QUALIFICATIONS & SKILLS • Familiarity of NYC public school system preferred, • Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred

    ESSENTIAL RESPONSIBILITIES The Bilingual Early Intervention ISC and OSC Service Coordinator’s responsibilities may include: • Serve as the primary Service Coordinator for assigned infants, toddlers, and families., • Explain the Early Intervention Program, service coordination process, family rights, procedural safeguards, and available services to families., • Communicate with families in their preferred language when qualified to do so., • Develop trusting, respectful, and family-centered relationships with parents and caregivers., • Assist families with navigating the Early Intervention system., • Coordinate and facilitate communication among families, evaluators, Early Intervention providers, Early Intervention Officials/Designees, and other professionals involved with the child., • Assist families with scheduling and coordinating multidisciplinary evaluations and other required appointments., • Participate in and coordinate IFSP meetings as required., • Assist families in identifying priorities, concerns, resources, and desired outcomes for their child and family., • Support the development, implementation, review, and amendment of the child’s IFSP., • Assist families with identifying available Early Intervention providers for authorized services., • Coordinate the initiation of authorized Early Intervention services., • Monitor whether services are being delivered in accordance with the child’s IFSP., • Follow up with families and providers regarding missed, delayed, interrupted, or unavailable services., • Identify barriers affecting service delivery and communicate concerns to the appropriate Early Intervention personnel., • Maintain ongoing contact with assigned families in accordance with program requirements and individual family needs., • Assist families with accessing community programs, healthcare resources, social services, educational resources, and other supports when appropriate., • Advocate for family participation and informed decision-making throughout the Early Intervention process., • Support transition planning as children approach age three and prepare to transition from the Early Intervention Program., • Collaborate with families regarding potential transition to preschool special education or other appropriate community services., • Complete all required case notes, service coordination documentation, contact logs, IFSP-related documentation, and other records accurately and within required timeframes., • Maintain accurate and current information within required Early Intervention systems., • Maintain confidentiality of child and family information in accordance with applicable federal and New York State requirements and Angel Community Alliance policies., • Participate in required orientation, training, supervision, quality-assurance activities, and professional-development activities., • Follow all applicable New York State Early Intervention Program requirements, policies, procedures, and ethical standards., • Bilingual Service Coordinators may be responsible for:, • Communicating directly with families in both English and their qualified second language., • Explaining Early Intervention processes and requirements in a manner families can understand., • Supporting culturally and linguistically responsive communication., • Helping reduce language-related barriers to participation in Early Intervention services., • Communicating effectively with bilingual families while maintaining professional boundaries and confidentiality., • Utilizing approved interpretation and translation resources when circumstances require services beyond the Service Coordinator’s language competency or role. MINIMUM QUALIFICATIONS Candidates must meet applicable New York State Early Intervention Program requirements for Service Coordinators and Angel Community Alliance credentialing requirements. Preferred qualifications include: • Bachelor’s degree or higher in psychology, social work, human services, early childhood education, special education, sociology, counseling, family studies, behavioral science, or a related field., • Experience working with infants, toddlers, children, or families., • Experience with children with developmental delays or disabilities., • Knowledge of Early Intervention, developmental disabilities, special education, Medicaid-funded programs, or community-based services., • Experience with case management or service coordination., • Strong written and verbal communication skills., • Strong organizational and time-management abilities., • Ability to manage multiple cases, deadlines, appointments, and documentation requirements., • Ability to communicate professionally with families, providers, government representatives, and other professionals., • Ability to work independently while remaining responsive to supervision and agency requirements., • Ability to maintain accurate, timely, and confidential records. Spanish-speaking candidates are highly preferred. Additional languages commonly spoken by families throughout New York City are also welcomed.
