The Emergency Department is divided into five major zones: Major trauma and critical care, Resuscitation and minor care, Pediatrics, Acute care, and Rapid Care. There are two distinct waiting areas, one for pediatric patients and their families, and the other for all other visitors. There are 7 negative flow rooms, one of which is also an isolation room. Clinical and clerical workstations, nutrition areas, and supply and pharmaceutical dispensing machines are centrally located in each zone. A dedicated radiology suite, including 2 CTs, a Decontamination suite with a separate entrance, as well as a satellite lab are also located within the Emergency Department. Able to work effectively within a team of multi-specialty professionals Job Description • Obtain and accurately record patient’s vital signs; reports abnormal values to treating provider, • Obtains and documents a patient’s history (Medical, Surgical and Social) and focused physical assessment, related to the clinic visit and as dictated by clinic procedure, • Administers medications and treatments to patients in clinic, under the direction of a physician or APP, • Participates in in-office procedure, as related to clinic specialty, under the direction and supervision of a Physician or APP and documents results in EHR, • Performs review and triage of incoming test results, patient requests and pharmacy renewals; facilitates call-backs to patients as necessary, • Facilitates the utilization of resources to meet patient outcomes and contribute to Facility (AMC), New York State and National Quality data metrics, • Delegates tasks deemed appropriate per licensure of LPN and/or MA, • Adheres to AMC’s regulatory agency (The Joint Commission) and internal compliance policies, • Orientation and education of newly hired staff, • Completes continuous/annual education related to Clinic specialty to maintain knowledge base on current practices, • Provides education to patients related to their visit and overall health and wellness Minimum Qualifications • Associate’s degree of a state accredited Registered Nursing program is required, • NYS current RN licensure to practice professional nursing is required, • Current CPR certification required, • Certification in specialty, Ambulatory setting preferred, • Excellent customer service and communication skills, • Ability to effectively present information and respond to questions from physicians, patients and their family members or other employees within the work setting. Overview Manages the development, operations and quality of the Nurse Family Partnership (NFP) team, ensuring fidelity to the NFP model is consistent with NFP national and Department of Health and Mental Hygiene (DOHMH) standards, and VNS Health philosophy, policies, goals and objectives, and Standards of Nursing Practice. Ensures that NFP and VNS Health standards for service delivery and team performance are met or exceeded. Promotes NFP Program through liaison, lecturing and promotional activities in the community., • Manages service delivery of assigned NFP team; ensures achievement of team goals and performance measures within the NFP and Agency’s strategic objectives and assumes accountability for achievement of team’s clinical, process and fiscal performance outcomes., • Observes and analyzes performance patterns and uses reflective supervision to develop interventions to improve individual and team performance. Provides guidance and support to assigned team to encourage and support team learning, implementation and evaluation of plans that maintain and increase customer response, cost-efficiency, and quality care compliant with NFP and regulatory standards., • Ensures that all staff, including supervisor, attend all required NFP Initial Education sessions as outlined in the orientation materials and contract with the Nurse-Family Partnership National Service Office., • Acts as a key resource and/or liaison in providing clinical and operational guidance and support to team. Uses NFP, DOHMH and VNS Health resources to achieve and enhance team outcomes. Supports, collaborates and provides guidance to NFP nurses in working through team development issues., • Develops staff through individual and team coaching, use of internal and external resources and participation in NFP education and training sessions to build confidence and skill of nurse home visits. Involves staff in the process of self-development and holds staff accountable for meeting performance expectations and development plans. Uses a variety of methods in obtaining information about team member’s performance (e.g., team meetings, home visits, frequent chart review, and feedback from other professionals and clients)., • Analyzes and helps to resolve complex clinical case management problems, working in collaboration with other NFP managers, clinical specialists, and other VNS Health professional staff, as appropriate. Provides guidance to individuals and team to identify and resolve internal and external customer issues., • In consultation with team members, identifies staffing and service needs of designated geographic areas and recommends changes as needed., • Interprets and implements NFP/DOHMH and agency policies, state and federal regulations, and collective bargaining agreements., • Collaborates with other NFP team members to develop and maintain relationships with community health and social service providers to ensure reciprocal referral relationships that support NFP enrollment requirements and access to resources that meet family needs., • Performs all duties inherent in a managerial role. Ensures effective staffing, evaluates performance, provides input for the development of the NFP budget, and hires, promotes, and terminates staff and recommends salary actions, as appropriate.
