Job Description Job Title: Wound Care Registered Nurse (RN) Location: Brooklyn, NY Shift: 8:00 AM – 4:30 PM (5 days/week, 8-hour shifts) Open Positions: 1 Pay Rate: $65–$69/hour Required Skills & Experience • Minimum two (2) years of Med/Surg experience, • Wound and Colostomy care experience required Required Education • Associate’s Degree in Nursing (ASN, ADN) from an accredited Nursing Program Required Certifications & Licensure • Active New York State RN License, • Primary Source Verification of License and Registration from NYS Department of Education (NYSED), • Current AHA BLS certification One of the following wound care certifications: • Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB, • Certified Wound Ostomy Nurse (CWON) – WOCNCB, • Certified Wound Care Nurse (CWCN) – WOCNCB, • Certified Wound Specialist (CWS) – ABWM, • Wound Care Certified (WCC) – NAWCO Job Summary The Wound Care RN conducts health assessments focused on wound, ostomy, and continence-related care. This includes performing focused physical assessments, developing individualized care plans, and recommending appropriate medications and treatments. Key Responsibilities • Develop, revise, and evaluate standards of care for wound and ostomy management, • Assess and document patient responses to wound, ostomy, and continence treatments, • Evaluate quality and delivery of patient care provided by clinical staff, • Collaborate with the healthcare team and refer to other disciplines as needed, • Educate patients, families, and staff on wound and ostomy care and complication prevention, • Develop and deliver staff education programs (orientation, in-services, continuing education), • Perform comprehensive assessments of patients at risk for pressure injuries and initiate preventive measures, • Identify and communicate challenges, including ethical and conflict-resolution issues, • Work with nursing leadership to implement evidence-based wound care practices, • Participate in unit and hospital-wide quality improvement initiatives, • Provide culturally competent education to patients, families, and staff, • Maintain confidentiality and data security in accordance with patient rights policies, • Pursue ongoing professional development to maintain clinical competency, • Identify and connect patients to community resources as needed, • Manage wound care–related equipment and assets (e.g., specialty beds, negative pressure devices)Company DescriptionWho We Are: Health Source Group Address: 25 Newbridge Rd, Hicksville, NY 11801, United States Founded in 2005, Health Source Group is a leading provider of healthcare staffing solutions.
