POSITION SUMMARY: Under the supervision of the Director of Community Housing and Alumni Support, the THP-Plus Case Manager is responsible for the evaluation, placement, and assessment of prospective and enrolled eligible young adults participating in the Transitional Housing Plus Program (THP-Plus). The target demographic includes young adults (18-24) who have been emancipated from foster care (including those supervised by the Juvenile Probation). The goal of the THP plus program is to offer housing and supportive services in a safe living environment, while helping participants develop the life skills needed to live independently. Program participants are eligible to participate for 36 cumulative months or until the age of 25, whichever comes first. THP-Plus Case Manager will work with young people to ensure success in securing stable housing, employment, education, health and wellness, and circles of support or permanent connections.
