Job Description Case Manager Job Description The role of the case manager is central to the day-to-day functioning of any residential program. This individual assumes primary responsibility for establishing the house routing; for orchestrating all house activities; for ensuring that the house itself is kept clean and in good repair; for ensuring that each resident attends all scheduled programs and appointments; for ensuring that all medications are ordered and refilled as necessary and for establishing the tone of the house. Checking to see if each resident has properly performed personal hygiene activities (showering, weather appropriate clothing, dressed in clean and neat clothing); checking cleanliness of all houses (kitchen, bathrooms, living space, bedrooms); assist residents in cleaning as needed;, 4. Using restorative services as a guideline to make sure that program activities are scheduled throughout the day and evening, which will allow every resident the opportunity to gain the skills necessary in order to move to the next level of independent functioning; make sure staff are assigned to carry out these activities and that residents are scheduled to involve themselves in them., 5. Document all conversations regarding each resident; make sure that all appointments, which are received verbally, are noted in the log book as well as a calendar for the day of appointment; make sure that all doctors orders for meds are faxed directly to the appropriate pharmacy; make sure contact info for all residents doctors, treatment teams, other providers are all up to date in system., 6. Provide 1-2 shopping trips a week for residents to buy their own household supplies and food items, 7. Plan, in concert with supervisor and other staff, for the conduct of evening and weekend activities such as shopping, field trips or purely recreational events; make sure funds are available to do such activities., 8. All rooms should be inspected for cleanliness and neatness; all rooms should be inspected to ensure that each contains all necessary items for daily living (sheets, towels, blankets, personal grooming, supplies), 9. All physical items in house should be in working order (kitchen appliances, bathrooms, all cooling/heating systems), 10. Possible Evening/Weekend coverage will be required. • The houses should be fully cleaned, residents must be assigned specific chores to ensure the cleanliness of house is maintained Other duties, as may be required, as situations arise throughout each day.

Essential Duties: • Engages residents in activities of daily living to the extent of their ability and supports individual needs in areas of shopping, meal preparation, cleaning, bathing, dressing, toileting, feeding, grooming and hygiene., • Supports individual health and service plans (HSP/ISP) or other goals and objectives; regularly observe, identify, document and report changes in health and / behavior., • Perform household chores as needed and assist residents to determine social, recreational, or educational activities of choice., • Adhering to standard health precautions, ensure well-being of residents by monitoring individual health, administering medication as prescribed., • Inspects individual equipment and furnishings for proper functioning and reports malfunctions to supervisor., • Ensure that safe and reliable transportation is available to residents. Drives van or other vehicle; escorts residents to scheduled programs or other activities, recreational opportunities, or medical appointments as assigned by supervisor., • Prepares and communicates written progress notes relating to medication administration, individual daily activities, medical emergencies and other related areas as required by supervisor., • Interacts with residents’ family members and friends, facilitate communications and engagement within the community., • Attends and participates in staff meetings may assist residents to participate in house meetings, • Performs other related tasks as required by Group Home Manager, • Maintain a clean and safe working environment; rotation of household chores and adhere to standard health precautions., • Adhere to all PILLAR policies and procedures. Qualifications: • At least 18 years of age (Insurance Purposes), • HS Diploma or GED, • 1-2 Years of successful employment with the same establishment or agency, • 1 year experience supporting adults/children with developmental disabilities preferred, • Successful completion of criminal background history and fingerprint examination, • Valid driver's license with fewer than 4 points at all times. Fox Trail Memory Care Living (MCL) is seeking an enthusiastic and motivated individual to join our team as a Certified Medication Aide (CMA) - this is NOT a position for a Certified Medical Assistant. CMA's are Care Partners for us. Care Partners are the heart and soul of our homes. The Certified Medication Aide is responsible for providing the highest degree of quality care and services to the residents (and their families) in our intimate home-like settings. Roles and Responsibilities: • Build meaningful relationships with our residents and their families as you provide assistance with activities of daily living, • Attend to individual care needs and get to know residents' unique preferences and personalities, • Attend to the individual needs of residents, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements, • Knowledge of the individualized wellness plan for residents and provide support to the residents according to their care plan. Contribute to the care planning process by providing the house with specific information and observations of the residents' needs and preferences, • Maintain the comfort, privacy, and dignity of residents in the delivery of services to them., • Interact with residents in a manner that displays warmth and promotes a caring environment, • Follow the directions of management and work as a positive team member of the Care Partner team (while using Fox Trail's strategy guides for success), • Work as a team to create meaningful activities for the residents, • Assist in maintaining a safe, neat and clean environment, • Promote a homelike environment for residents, • Cook healthy meals in conjunction with the menu, • Distribute residents' medication in an accurate and timely manner, • Dedication to and passion to serve seniors with excellent customer service skills, • Flexibility and a willingness to modify role duties for the overall good of the community, • High School diploma/GED, • State Required Certification or License for the position you are seeking, • Must be at least 18 years of age, • Previous experience working with seniors preferred, • Ability to make decisions and act in the resident's best interest, • Possess written and verbal skills for effective communication and a level of understanding, • Competent in organizational and time management skills, • Demonstrate good judgment, problem solving and decision-making skills, • Must be willing able to lift over 75 pounds (i.e. transferring of residents) Job Posted by ApplicantPro
