Your journey matters to us. 1115 Waiver Enhanced Navigator – Catholic Charities of Fulton and Montgomery Counties The 1115 Waiver Enhanced Navigator plays a crucial role in supporting individuals as they navigate available services, helping them access life-changing resources and empowering them on their journey toward wellness and stability. In this role, you will: • Conduct screenings with Medicaid-eligible individuals using the approved screening tools — ensuring accuracy, compassion, and proper documentation., • Enter and maintain timely, accurate data in the Unite Us platform, meeting all 1115 Waiver requirements., • Educate clients, referral partners, and community members about waiver eligibility and available services., • Participate in required trainings, including Unite Us, trauma-informed care, HIPAA, and compliance.
