Case management services are provided to community members impacted by intentional violence as they heal from injuries post-discharge from the hospital setting. Case managers are also essential to establishing relationships and partnerships with local treatment organizations that may serve those impacted by gun violence in Delaware County. Case managers focus on supporting discharge concerns with hospital staff and potential clients, connecting clients to resources, supporting clients in meeting their goals to mitigate the impact of violent trauma on their lives, and maximizing satisfaction with the outcomes of their participation in Chester Community Coalition programs. Job Description POSITION: Program Manager (SEASONAL) REPORTS TO: Program Director POSITION SUMMARY: This position is part of a seasonal team responsible for the Urban Affairs Coalition's Summer Youth Employment and Spark Youth Engagement (SYE) program. The Summer Youth Employment program provides paid summer internships and support to youth ages 14-24 through the city of Philadelphia’s summer youth employment program. The six-week program runs from late June through mid-August and includes career exposure, on-the-job mentoring and professional development. The Spark Youth Engagement Program offers career exposure and career awareness activities for in school youth and is offered year-round. Initiatives under Spark Youth Engagement include career exposure, professional development, and mentoring with the goal of entering the workforce. UAC anticipates hiring three Program Managers. With 280 program sites — concentrated in Philadelphia, Pittsburgh, and the central corridor of the state — we distribute books and train pediatric providers to promote reading aloud with young children during well-child visits. Our work spans urban health systems to single-provider rural practices, serving children from diverse communities including those from low-income families and medically underserved regions. This is a newly created position, reflecting a period of significant growth and investment for the Pennsylvania affiliate. As Program Manager for Central and South-Central Pennsylvania, you will serve as a primary point of contact for approximately 85–100 clinical sites across a defined geographic territory. You will build and sustain relationships with medical providers, assess program quality and fidelity, and help sites thrive through hands-on support and creative problem-solving. This role reports directly to the Executive Director of Reach Out and Read Pennsylvania and is a critical piece of the affiliate's expanding regional structure.
