The Embedded Peer Specialist provides peer support for each participant on the Rockland County Psychiatric Unit and in the Emergency Department. The Peer is available to instill hope, support wellness and demonstrate that recovery is real and does happen. Peers model and utilize trauma-informed techniques and tools, help participants set goals and engage in community and social events.Duties and Responsibilities: Provide support for participants to divert hospitalization after discharge. Share personal experience and encourage mental health wellness and recovery. Assist in connection with mental health services, community resources, entitlements, and other services as needed. Schedule for Behavior Specialist: Sunday thru Thursday from 10:00am until 6:00pm Essential Duties of Behavior Specialist Key duties of this position include, but are not limited to, the following: • Understanding and supporting the agency’s mission & treatment philosophy; Supports overall strategic plan of the organization., • Treats all children, families, and co-workers with respect & courtesy; Demonstrates & models’ sensitivity to cultural background of children, families, and co-workers; Honors Sanctuary Commitments in all day-to-day activities., • Effectively manages behavioral interventions for residential children:, • Completes and implements comprehensive Functional Behavior Assessments (FBAs) and Behavior Intervention Plans (BIPs) integrating principles of Applied Behavior Analysis (ABA), Cognitive Behavioral Therapy (CBT), and trauma-informed care., • Collects/records behavioral data, interprets results, and updates/modifies BIPs accordingly., • Learns and coaches students in social skills, emotion regulation, and body regulation strategies., • Supports interdisciplinary care team to understand behavioral data and implement BIPs effectively., • Identifies target behaviors for children and collaborates with interdisciplinary treatment team to address them through interdisciplinary interventions., • Assists Residence Manager in developing effective Positive Behavioral Intervention and Support (PBIS) interventions., • Helps team collect, maintain, and interpret data for effective treatment planning., • Facilitates and collaborates with the residential team to conduct social and life skills groups, weekly residential groups, and restorative circles., • Works effectively with interdisciplinary team members and families:, • Collaborates respectfully with parents and family members, holding restorative conversations to rebuild relationships and understand the impact of incidents., • Works closely with psychologists, Occupational Therapists (OTR), clinical social workers, and program managers to ensure methodologies align and support each other., • Completes documentation requirements timely and ensures reporting is clear, accurate, and timely., • Tracks, communicates, and documents integrity checks of BIPs and target behaviors systematically, in conjunction with the treatment planning team., • Utilizes technology to track positive changes in behavior., • Integrates self-regulation antecedent and consequence strategies into BIPs., • Documents services and programs related to Activities of Daily Living (ADLs) and tracks ADL data over time., • Ensures program-wide material is visual, clear, and developmentally appropriate., • Supports the overall strategic plan of the organization;, • Ability to adhere to the organization’s core values and commitments. Minimum Qualifications of Behavior Specialist • Bachelor’s degree in psychology, Counseling, Education, or a related field, • At least one year of experience working in a behaviorist role, • Experience working with children, and/or vulnerable populations, • Knowledge of behavioral intervention techniques and strategies, such as Applied Behavioral Analysis (ABA) principles, • Ability to collect, record, and analyze behavioral data., • Strong communication and interpersonal skills to work effectively with clients, families, and interdisciplinary team, • Ability to access and utilize ADP app on smartphone or other device for access to agency programs This job description in no way states or implies that these are the only job duties to be performed by the incumbent. We specialize in providing quality coordination of care, referrals, peer support, groups, and other related services in a friendly environment. Purpose of the Role: The Street Outreach Specialist is part of a coordinated outreach team that identifies and engages people living in unsheltered locations, such as in cars, parks, abandoned buildings, encampments, and on the streets. To include outreaching shelter homeless individuals and those at risk of becoming homeless. The outreach team reaches people who might not otherwise seek assistance or come to the attention of the homelessness service system. Through its work, the team ensures that people’s basic needs are met while supporting individuals along pathways toward housing stability and quality of life improvements. The Outreach Specialist is responsible for conducting outreach, providing information and referrals, completing assessments, intakes, performing short-term case management, and delivering concrete services. Essential Functions of the Role: • Identify and build relationships with organizations and individuals in places such as, but not limited to hospitals, shelters, partner agencies, medical care clinics, mental health providers, soup kitchens, food pantries, associations dedicated to helping marginalized people, etc., promoting and building awareness regarding CHOICE, its mission and services., • Co- respond to crisis calls with local government agencies, • Outreach identified “hot spots” at times when individuals are most likely to be found (early morning including some evenings), • Using a trauma-informed approach, provide crisis intervention, harm reduction, and culturally Responsive competent engagement techniques., • Meet one-on-one with a person needing services, screen to determine eligibility., • For persons eligible to receive benefits and become a client of CHOICE, remain in close partnership with them, monitoring the enrollment process, until intake has been completed., • Act as a support partner to a potential client, assisting them with their essential care relationships through the completion of the enrollment, • Review with Outreach Supervisor (or appointed staff person) for final approval, persons that have been identified as a potential client and can benefit from CHOICE., • Complete street outreach assessment (VI-SPDAT) as requested by HOW Supervisor, • Document all outreach efforts in HMIS (Homeless Management Information System), Relevant and Awards database., • Attend meetings, as necessary, at the Department of Community Mental Health, to review client cases. For all fieldwork: • Conduct daily street outreach and respond to calls for assistance, • Conduct a minimum of 20 intakes per a week, • Proactively report to the Homeless Outreach Supervisor all trends, relationships, and developments in the field., • Track activities and outcomes.
