Job Summary and Qualifications Performs the primary function of a professional leader in assessing, planning, organizing, directing and evaluating program activities on a 24-hour basis, in accordance with facility policies, philosophy, objectives and standards. The position is responsible for managing patients with stroke and transient ischemic attacks, stroke interventions and procedures, sepsis and cardiovascular procedures. The Coordinator is responsible to ensure quality in all areas of education, patient care, patient management and performance improvement activities. The coordinator functions as a resource, and role model, patient advocate and provides up-to-date information for staff, patients and families to enhance quality patient care. Assures needed supplies and equipment available for nursing staff by working with the Nursing Managers and Nursing Directors. Promotes teamwork with physicians, staff, and patients and promotes excellence in care through education and mentoring. Maintains a well-equipped working environment and provides continuing growth opportunities that allow for the achievement of maximum productivity and satisfaction. Leads and coaches the development and growth of a safety culture in the hospital. What you will do in this role: • Achieves professional goals set during previous period, • Demonstrates professional behavior and provides leadership support for staff, • Assists in development, managing, ensuring compliance with TJC Disease Specific Certification Primary Stroke standards, CMS Core Measures (Stroke and Sepsis) and Get with the Guidelines (Stroke) and assists with improving outcomes with heart surgery, stroke and sepsis, • Works cohesively with the Medical Directors of Neurovascular Department and Cardiovascular/Surgery to serve as a liaison to facilitate goals in alignment with Hospital and Corporation, • Demonstrates accountability for assessing, planning, directing and evaluating care of the patient with a stroke or TIA, and post op neurological surgery, severe sepsis/septic shock and cardiac surgery, • Demonstrates good communication and customer skills with patients, families, physicians, peers and healthcare teams., • Participates and facilitates discharge planning, decrease excess days, cost containment and assist with decreasing readmissions with stroke, sepsis and cardiac surgery, • Development and facilitates Performance Improvement activity, • The Stroke Coordinator is fundamental to the development, implementation and evaluation of the stroke program directed towards the provision of quality services for patients who are experiencing stroke from a multi-disciplinary perspective throughout the continuum of care, • Works collaboratively with the Stroke Program Medical Director in a mutually supportive relationship which is essential to the success of the program, • Provides leadership for the organization of services and systems necessary for a collaborative approach to stroke care. The Stroke Coordinator will assume the day-to-day responsibility of process and performance improvement activities as they relate to nursing and ancillary personnel., • Plans, implements and evaluates clinical programs to achieve high quality, patient-focused outcomes which advance patient and family care and nursing practice in stroke care, • Responsible for the Stroke Program development, quality assessment, improvement along with interdisciplinary communication., • Works collaboratively with all hospital and clinic personnel on shared system and quality improvement projects.
