Key Responsibilities • Serve as the agency’s designated Director of Patient Services in accordance with NYS Department of Health regulations, • Oversee and manage all patient care services and clinical staff, • Ensure compliance with state, federal, and agency policies and procedures, • Supervise, train, and evaluate nursing and clinical personnel, • Conduct patient assessments, care plan reviews, and case conferences, • Monitor quality assurance and performance improvement programs, • Ensure proper documentation and regulatory reporting, • Collaborate with management, caregivers, and referral sources, • Participate in audits, surveys, and inspections as required, • Support agency growth and patient satisfaction initiatives Qualifications • Current, active Registered Nurse (RN) license in New York State (Required), • Eligible to serve as DPS under NYS regulations (Required), • Minimum of [2–3+] years of nursing experience, preferably in home care or community health, • Prior supervisory or management experience preferred, • Strong knowledge of NYS home care regulations, • Excellent communication and leadership skills, • Strong organizational and time-management abilities, • Proficiency with electronic medical records (EMR) and EVV systems preferred Compensation & Benefits • Competitive salary based on experience, • Hourly and per visit rates, • 401k, • Paid time off, • Professional development opportunities, • Supportive and collaborative work environment
