Our team provides comprehensive primary care for the whole family — from pediatric patients (ages 5 and up) through adults and seniors — with same-day appointments and convenient telehealth visits available. Our practice goes beyond episodic care. We treat the full range of primary-care needs, including annual physicals and preventive screening, women's health, men's health, pediatric care, diabetes and hypertension management, lipid and cardiovascular risk reduction, mental and behavioral health, and medically supervised weight-loss and metabolic-health programs. We accept most major insurance plans, including Medicare, and care for patients across commercial, Medicare, and value-based arrangements. Essential Duties and Responsibilities Direct Patient Care • Provide comprehensive primary care services to a panel of pediatric (ages 5+), adolescent, adult, and geriatric patients, including acute, chronic, and preventive care., • Perform thorough history-taking, physical examinations, and age-appropriate screening; order and interpret diagnostic and laboratory tests; diagnose and manage acute and chronic conditions including diabetes, hypertension, hyperlipidemia, and other common primary-care conditions., • Provide women's health and men's health services, including annual exams, screening, and routine reproductive and sexual health care within scope of practice., • Support CornerstoneMD's medically supervised weight-loss and metabolic-health programs, including counseling, lifestyle interventions, and pharmacologic management., • Conduct telehealth visits for established and eligible new patients in accordance with practice protocols and applicable law., • Prescribe medications, including controlled substances to the extent authorized by APP's individual prescriptive authority, DEA registration, and the Practice Agreement., • Perform in-office procedures consistent with training and scope of practice (e.g., joint injections, skin biopsies, incision and drainage, IUD placement, suturing) as credentialed by the practice., • Provide patient and family education, motivational interviewing, and counseling on lifestyle modification, medication adherence, and disease self-management., • Refer to specialists when clinically indicated; coordinate follow-up and close referral loops., • Support same-day and next-day patient access for urgent primary-care needs. Panel and Population Health Management • Take ownership of an attributed patient panel and build long-term, trusted relationships with patients and families., • Conduct Annual Wellness Visits, Transitional Care Management visits, Chronic Care Management check-ins, and Advance Care Planning conversations., • Identify and close care gaps and contribute to accurate clinical documentation and coding., • Support proactive outreach to high-risk and rising-risk patients; participate in interdisciplinary huddles and case conferences with the care team., • Coordinate care across hospital, SNF, home-health, and specialist settings to support continuity and reduce avoidable utilization. Documentation, Billing, and Compliance • Document all encounters in the electronic health record completely, accurately, and in a timely manner, in accordance with CornerstoneMD policies, payor requirements, and applicable law., • Comply with all applicable federal and state laws and regulations, including HIPAA, the Anti-Kickback Statute, the Stark Law, the False Claims Act, Texas scope-of-practice rules, and the rules of the Texas Board of Nursing or Texas Medical Board (as applicable to APP's licensure)., • Adhere to CornerstoneMD's “incident-to” and split/shared-services billing protocols where applicable., • Maintain the written Practice Agreement (collaborative practice agreement, delegation of services agreement, or supervisory agreement, as applicable) on file at CornerstoneMD; meet regularly with the Collaborating/Supervising Physician as required by law and policy. Team and Practice Citizenship • Collaborate respectfully and effectively with physicians, fellow APPs, RNs, medical assistants, care coordinators, and administrative staff., • Participate in clinical quality improvement initiatives, peer review, chart review, and CME activities., • Help train and mentor medical assistants, students, and new team members as opportunities arise., • Maintain a professional, patient-centered demeanor; uphold CornerstoneMD's reputation in the community. Required Qualifications Education and Licensure • For Nurse Practitioner candidates: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program; current unrestricted Texas APRN license with Family Nurse Practitioner (FNP) population focus (or other primary-care population focus relevant to the role); current national certification through AANP or ANCC; Texas prescriptive authority and full prescriptive authorization agreement-readiness., • For Physician Assistant candidates: Master's degree from an ARC-PA accredited PA program; current unrestricted Texas PA license issued by the Texas Medical Board; current NCCPA certification (PA-C)., • Current, unrestricted DEA registration (or eligibility to obtain one prior to start date), including Schedule II authority where permitted., • Current BLS certification Experience • New graduates with strong primary-care training are encouraged to apply; 1–3+ years of outpatient primary-care or family-practice experience preferred., • Experience caring for diverse adult and pediatric populations across commercial, Medicare, and value-based payor mixes is a plus., • Comfort with electronic health record systems, telehealth platforms, and routine in-office point-of-care procedures. Preferred Qualifications • Bilingual (English/Spanish) preferred but not required., • Familiarity with quality, gap-closure, and risk-adjustment documentation., • Experience caring for geriatric populations and managing multiple chronic conditions., • Experience supporting weight-loss / metabolic-health programs (including GLP-1 therapies)., • Prior experience working under a collaborative practice agreement or delegated supervisory model in Texas. Skills and Competencies • Sound clinical judgment within scope of practice; intellectual honesty about the limits of one's knowledge and willingness to consult., • Strong written and verbal communication; ability to explain diagnoses and treatment plans clearly to patients and families with varying health literacy., • Efficient time management and the ability to maintain quality and documentation standards in a busy primary-care setting., • Collaboration and humility — the willingness to work closely with physicians, peers, and the broader care team., • Comfort with quality dashboards and clinical performance reporting., • Discretion, confidentiality, and adherence to professional and ethical standards.
