Job Responsibilities: • Priority will be given to the completion of the Pre-and-Post Parenting Skills Knowledge survey to ensure the program attains the contracted number of outcomes each year., • Utilize a case management model for participants in all tiers, which would include the following 4 components: (1) holistic assessment of the father’s strengths and needs and development of an Individual Service Plan (ISP) listing supportive services, benefits and resources needed; (2) implementation of the ISP (coordinating community resources and working with and advocating on behalf of the family to meet identified goals); (3) regular review of the ISP to assess whether goals have been met or need to be changed; and (4) follow-up to ensure that the father has received requested services.
