How YOU will benefit • Provide 1:1 care to make a lasting impact on patients and families, • Greater work/life balance with flexible scheduling options, • Less time on your feet compared to other settings, • Ability to work independently while also having team support, • Job stability and regular advancement opportunities with a growing company As a Hospice LPN You will: • Give hands-on care to patients nearing the end of life, following state nursing rules., • Follow the care plan after assessing the patient’s needs., • Keep clear and timely records of the care provided., • Teach and support patients and their families about the illness and care plan., • Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers)., • Manage pain and symptoms, checking how well treatments are working., • Take part in training and quality improvement activities to grow professionally and improve care. Community health/hospice or medical/surgical experience is preferred., • Currently licensed as an LPN/LVN in the State., • Comply with accepted professional standards and practice., • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order., • Possess and maintain current CPR certification., • Understand philosophy of hospice concept. Also understand needs of the terminally ill., • Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems., • Demonstrate good verbal and written communication, and organization skills.
