The ideal candidate will have experience in an integrated team approach in providing in home care management services and supportive counseling for Hoarding Support Program clients, ages 18+. The SW/CM identifies the needs of clients and provides hoarding case management services and facilitates Buried in Treasures support groups. This position provides in-home, direct services for clients who are experiencing hoarding behaviors. Services may also be provided in the Northeast HUB (northeast), OIC Philadelphia (north), Brodsky Center (Bala Cynwyd) offices and in-home. Groups may be provided via telehealth or in person. The Social Worker will play a crucial role in supporting clients' needs while ensuring compliance with HIPAA regulations. Travel is a must. Duties and Responsibilities • Coordinates and provides supportive counseling and care management services for clients with hoarding disorder in the client's home. There is a large proportion of time spent in clients' homes for this duty. The average caseload of individuals clients will be between 14-20 depending on geographic area, we serve the 5-county area., • Conducts comprehensive assessment of clients' needs and assesses their goals to develop mutually agreed upon treatment/care plans., • Works with clients to enable access to resources appropriate to their individual needs (e.g., referrals to legal services, mental healthcare, and increasing supports in the home)., • Collaborates with community resources and assists clients with daily life skills to help them function at as high a level as possible., • Maintains knowledge of a broad variety of community resources and refers clients when appropriate to support their treatment goals., • Provides Motivational Interviewing and Cognitive Behavioral Therapy skills to clients with hoarding behaviors including direct support with sorting and discarding.
