Position Overview: Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards. Requirements Qualifications: • Education: ADN/BSN from an accredited program, • Licensure: Current and active New York State Registered Nurse (RN) license is required., • Certification: Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment., • Experience:, • Must have Pediatric experience., • Minimum of 1.5 years of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity., • Proven experience in hospital case management within an acute care environment is essential., • Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system is preferred.
