In collaboration with the interdisciplinary care team, they will provide assessments, advocacy, and comprehensive care coordination services to our highest-risk patients and their caregivers to help them improve their health status and avoid hospitalizations. Core Responsibilities: • Bilingual Spanish Required, • Proactively support our highest-need patients by providing comprehensive assessments, telephonic check-ins, and in-person visits., • Partner with the patient and their support persons (if necessary) to identify goals, increase engagement in their healthcare, address barriers, and drive the execution of their care plan., • Maintain extensive knowledge of community resources and make referrals, as appropriate, to help ensure the patient's healthcare and psychosocial needs are met., • Navigate patient insurance benefits in collaboration with the Patient Relations Manager (PRM) to ensure mindful utilization and maximization of plan benefits, • Provide both longitudinal and episodic comprehensive care coordination with the ultimate goal of empowering patients to independently manage their health conditions and have increased quality of life., • Educate patients on supportive care options (including advance care planning and end-of-life care) and provide goals of care counseling, support, and advocacy at all stages of the patient journey., • Provide psychoeducation to patients and families surrounding life stage and disease-specific issues, including dementia, adjustment to illness, aging, caregiver support, etc., • Meet with care teams to collaborate and generate creative solutions to patient issues, and spearhead intervention in situations with complex psychosocial dynamics., • Complete concise and comprehensive documentation on time to ensure that all care team members have visibility into the status of MSW interventions, • Partner with Community Health Workers and provide clinical guidance and consultation regarding complex patient needs to these team members., • Respond to crises and urgent needs with the keen ability to prioritize work, triage situations, and multitask., • Other duties as assigned. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Overview Provides targeted social work services to patients at high risk for rehospitalization during a transitional care episode in the Transitional Care Program. Functions as part of a social worker-led team providing services to this population. Works under general supervision. Overview Provides cross-program clinical leadership and support across seven Intensive Mobile Treatment (IMT) teams. Partners with the Associate Director of Treatment Programs and team managers to promote consistent clinical practice, high-quality service delivery, staff development, and adherence to program, contractual, regulatory, and organizational requirements. Serves as a clinical resource to multidisciplinary teams working with adults who have complex behavioral health, substance use, medical, housing, and justice-system needs in community-based settings. Supports clinical assessment, case formulation, engagement, treatment and recovery planning, risk assessment and management, crisis response, documentation, and care coordination. Conducts regular team visits, case consultations, chart reviews, and observations of team practices. PURPOSE: The Center for Jewish Mental Health provides culturally responsive outpatient mental health care within the Seymour Askin Counseling Center/Brooklyn CCBHC. This position includes a defined responsibility to provide individual and group psychotherapy through The UJA Funded - Lev Initiative, which serves Jewish survivors of intimate partner violence and their families. The clinician will also maintain a broader outpatient caseload serving clients with diverse backgrounds and clinical needs. POSITION OVERVIEW: The social worker will provide assessment, treatment planning, psychotherapy, crisis intervention, care coordination, and documentation in accordance with agency and regulatory standards. A portion of the role will focus on Lev Initiative clients, including individual and group services for survivors of intimate partner violence and, when clinically appropriate, affected family members or children. Services should be trauma-informed, culturally responsive, and coordinated with relevant community, legal, housing, safety-planning, and clinical supports. The role is not limited to Lev clients; the clinician may also serve a diverse outpatient caseload. Kaplan House offers an extensive independent living skills program, including vocational preparation, and educational services that assists youth towards reaching independence. POSITION OVERVIEW: The Clinical Director has full responsibility for the ongoing clinical and administrative guidance of the clinical staff. She/He also provided ongoing training, supervision and guidance to the clinical staff. The Clinical Director is the ITM (Integrated Treatment Model) liaison for the program. PURPOSE OF POSITION: The Trauma Program Social Work III is an integral member of the multidisciplinary Trauma team and provides comprehensive psychosocial assessment, crisis intervention, patient and family support, resource coordination, behavioral health linkage, longitudinal follow-up, and discharge planning for injured patients across the continuum of care. The Trauma Program Social Worker will follow all trauma patients from admission through discharge, beginning in the Emergency Department and continuing with patients admitted to inpatient units, step-down units, and Intensive Care Units (ICUs). The Social Worker will screen trauma patients from admission, conduct ongoing chart reviews, identify psychosocial and behavioral health needs, coordinate interventions and referrals, and provide continuity of psychosocial care throughout the patient's hospitalization and following discharge. The Social Worker collaborates closely with Trauma Surgery, Emergency Medicine, Nursing, Utilization Management, Rehabilitation, Behavioral Health, Addiction Services and other clinical services to address psychosocial barriers that may affect recovery, safety, adherence, disposition, and reintegration into the community. The position supports trauma-informed, culturally sensitive, patient- and family-centered care and contributes to the Trauma Program's performance improvement, injury prevention, education, community outreach, longitudinal follow-up, and trauma verification-readiness activities. Safety, Violence, and High-Risk Needs · Screen and respond to concerns involving interpersonal violence, intimate partner violence, community violence, exploitation, homelessness, and other high-risk social circumstances. · Coordinate mandated reporting when required by law and institutional policy. · Facilitate referrals to appropriate hospital-based and community violence intervention, victim assistance, behavioral health, shelter, legal, and social service resources. · Collaborate with the Trauma Program's injury prevention and outreach activities to identify opportunities for secondary injury prevention and risk reduction. · Address additional needs of vulnerable populations such as the elderly and youth. · Collaborate with Geriatrics and Palliative Care to address goals of care and advance directives. · Collaborate with community violence prevention resources at Lincoln Medical Center, including Lincoln Victim Assistance Program, when appropriate. Substance Use and Behavioral Health · Work with Addiction Services to identify trauma patients who may benefit from substance-use assessment, brief intervention, treatment referral, or behavioral health evaluation in accordance with institutional protocols. · Review Addiction Counselor/Addiction Services documentation and incorporate identified substance-use treatment needs into the patient's trauma psychosocial plan. · Provide motivational, supportive, and trauma-informed interventions within professional scope. · Coordinate referrals to Addiction Medicine, Behavioral Health, social service agencies, and community treatment resources as appropriate. · Assist with continuity planning for patients with pre-existing psychiatric or substance-use treatment needs. · Identify trauma patients requiring additional behavioral health intervention based on psychosocial assessment, clinical presentation, screening results, and established Trauma Program criteria. · Provide education regarding the emotional and psychological effects of traumatic injury, including common trauma responses and Post-Traumatic Stress Disorder (PTSD). ITSS Screening, Mental Health Referral, and Trauma-Related Behavioral Health Follow-Up · Review trauma patients for behavioral health needs utilizing established trauma screening tools and Trauma Program criteria. · Review ITSS (Injury Trauma Screening Score) results for trauma patients and identify patients who meet established criteria for additional psychosocial or behavioral health follow-up. · Follow trauma patients who meet established ITSS criteria and ensure that they are appropriately connected with mental health services. · Coordinate referrals to Behavioral Health, outpatient mental health providers, community mental health agencies, or other appropriate services based on identified needs. · Track mental health referrals and assist patients in overcoming barriers to establishing or maintaining behavioral health services. · Conduct follow-up with patients identified through ITSS screening to assess whether they successfully connected with recommended services. · Communicate identified behavioral health concerns and referral needs to the appropriate members of the interdisciplinary Trauma team. PTSD Education and Patient Support · Develop, maintain, and provide educational materials regarding PTSD and the psychological effects of traumatic injury to trauma patients and families. · Educate patients and families regarding common emotional responses following traumatic injury, coping strategies, warning signs of psychological distress, and when to seek additional mental health support. · Provide trauma-informed education regarding available behavioral health and community resources. · Develop and facilitate a Trauma Patient Support Group to provide education, peer support, coping resources, and opportunities for trauma survivors to address the emotional and psychosocial impact of traumatic injury. Discharge Planning and Continuity of Care · Participate in interdisciplinary discharge planning for trauma patients with complex psychosocial needs. · Coordinate with Case Management, Rehabilitation, Nursing, physicians, therapists, community agencies, and families to support safe and timely disposition. · Collaborate with outpatient nursing services for home assessment and PT in elderly trauma patients. · Assist patients and families with access to community resources, transportation, housing resources, benefits, disability-related resources, victim compensation resources, and other services when applicable. · Support transitions to rehabilitation facilities, skilled nursing facilities, home services, shelters, or other appropriate settings in collaboration with the responsible care-management team. · Promote continuity with trauma clinic, primary care, specialty care, rehabilitation, behavioral health, and community services. · Ensure that psychosocial, behavioral health, substance-use, and safety concerns identified during the hospitalization are communicated to appropriate providers and incorporated into the discharge and follow-up plan. Three-Month and Six-Month Trauma Patient Follow-Up · Conduct structured three-month and six-month follow-up with eligible trauma patients following discharge. · Assess the patient's psychosocial adjustment following traumatic injury, including emotional well-being, PTSD symptoms or concerns, mental health service connection, substance-use treatment needs, social supports, functional adjustment, and community reintegration. · Assess barriers to ongoing recovery, access to services, transportation, housing, employment, benefits, rehabilitation, primary care, behavioral health, and other community resources. · Determine whether patients identified through ITSS screening successfully connected with recommended mental health services. · Provide additional referrals and resource coordination when ongoing needs are identified during follow-up. · Document all three-month and six-month follow-up contacts in the electronic medical record in accordance with Trauma Program requirements. · Complete a report on three-month and six-month follow-up activities and findings, including identified psychosocial needs, referrals, behavioral health connections, barriers to care, patient outcomes, and opportunities for program improvement. · Identify trends in long-term psychosocial needs and communicate findings to Trauma Program leadership and the interdisciplinary team. This position ensures that members have a scheduled appointment in place, completes follow-up and reminder call to member and follows up with providers to ensure member’s compliance with aftercare. This is a full-time, fully in-person position and a three-year, grant-funded role, with current funding in place for the full grant period. The psychotherapist will be principally responsible for providing individual psychotherapy, completing clinical evaluations as part of New York City's 2010e supportive housing process, and leading clinical support groups. At launch, this role is grant-funded rather than insurance-supported and is not initially tied to insurance billing productivity requirements. Organization Overview: Founded in 1982, Broadway Community’s mission is to serve as a bridge from crisis to stability for New Yorkers experiencing homelessness, hunger, mental illness, and social isolation.
