Job DescriptionClosed Loop Care Manager (Remote) Location: Fully Remote (U.S.-Based) Schedule: Monday - Friday | 9:00 AM - 5:30 PM EST Compensation: $21-$24/hour Employment Type: 6-12 month contract with potential for conversion (Based on performance) About the Opportunity We are partnering with a rapidly growing healthcare organization supporting Medicaid populations through an innovative care management model focused on health-related social needs (HRSNs). This role goes beyond traditional care coordination by addressing non-medical factors that impact overall health outcomes - including food access, housing stability, transportation, and financial barriers. This is a highly outreach-driven role where you will engage members directly, coordinate services, and ensure completion of care plans through a closed-loop care model. Key Responsibilities Screening & Enrollment • Conduct outbound outreach to Medicaid members via phone, • Perform initial screenings to identify unmet social and health-related needs, • Guide members through enrollment in available support programs, • Gather and document key information to connect members with appropriate services Care Coordination & Member Engagement • Manage referrals, care plans, and service coordination across providers and community-based organizations, • Maintain consistent outreach via phone, text, and digital platforms to ensure member engagement, • Identify and address barriers to care including food insecurity, housing challenges, and transportation limitations, • Educate members on care plans, available resources, and next steps, • Support navigation of healthcare systems and community services Care Management (Closed-Loop Model) • Track referral status and ensure completion of services, • Follow up with providers and partners to confirm outcomes and next steps, • Escalate delays or gaps in care as needed, • Maintain accurate and compliant documentation in care management systems, • Monitor key performance metrics including outreach, engagement, and care gap closure, • Collaborate with interdisciplinary teams to support quality outcomes What You'll Be Working On • Supporting Medicaid members with whole-person care needs, • Addressing social determinants of health (SDOH), • Coordinating services such as:, • Nutrition and food access support, • Housing and utility assistance, • Transportation services, • Community-based programs Required Qualifications (Non-Negotiables) • Care management, case management, or care coordination experience, • Experience working with Medicaid populations, • Strong communication skills with ability to conduct high-volume outreach, • Ability to work fully remote with:, • Personal laptop (temporary use; no Chromebooks), • Reliable high-speed internet with Ethernet connection, • Quiet, distraction-free workspace Preferred Qualifications • Experience with social determinants of health (SDOH) or community resource coordination, • Background in healthcare, behavioral health, or social services, • Bilingual (Spanish) is a plus, • Familiarity with EMR/CRM systems and documentation standards What Makes This Role Unique • Fully remote opportunity with a mission-driven healthcare organization, • Direct impact on underserved populations, • Blend of care management + social services + outreach, • Opportunity to grow into a long-term role through contract-to-hire Next Steps If you're passionate about supporting individuals beyond clinical care and enjoy a fast-paced, outreach-focused environment, we'd love to connect with you!
