PURPOSE: The Mishkon Division provides supports and services in a healthy and safe environment for individuals with intellectual/ developmental disabilities living in residential programs and in community settings. This is all accomplished through an Orthodox Jewish lens, and with an understanding that staff, at all levels of the division, is the determining factor in achieving positive outcomes for our clients. The Behavior Intervention Specialist (BIS) provides behavioral assessment, intervention, and support to individuals with intellectual and developmental disabilities (I/DD) under the Office for People with Developmental Disabilities (OPWDD) in New York State. The BIS develops and implements Positive Behavior Support (PBS) plans, conducts Functional Behavior Assessments (FBAs), and trains staff/caregivers to ensure individuals receive effective, person-centered behavioral services in residential, day habilitation, or community settings. POSITION OVERVIEW: The Behavior Specialist, Level 1 will design, implement and monitor the Behavior Management Programs, Instructional Goals and Valued Outcomes for all individuals using the Positive Approaches Paradigm. This individual must be familiar with OPWDD and DOH regulations as they apply to the Intermediate Care Facility (ICF) and Independent Residence Alternative (IRA) and Community Services, such as Day Habilitation. The Behavior Specialist, Level 1 will have an assigned caseload of up to 35 individuals and will assist in coverage, as needed. Travel to units, IRAs, day programs and in the community is routine and required. Responsible for communication with and training the interdisciplinary team, outside providers, and family members. There is walking in the residential programs up and down stairs and at times the ability to lift a social service chart/field., • Travel: May require travel throughout Brooklyn., • BIS must work in person 5 days a week, • Typical schedule is active business hours of Mon-Friday 9-5, however there is ability for flex schedule based on the needs of the program and the caseload with one early start day and one late day to accommodate breakfast and dinner observations. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Under the supervision of the Medical Director of Mental Health or a supervising Psychiatrist in the Division of Correctional Health Services, this full-time position requires the provision of high-quality, team-based psychiatric care, including psychotherapeutic and psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. The psychiatric service strives to provide safe, holistic, and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units (“mental observation units”) for individuals with serious mental illness, a 24-hour outpatient-level clinic in each facility, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. Each of the 12 facilities in the jail system has unique patient population and environmental characteristics; each Psychiatrist will be matched with the population that most closely meets their clinical expertise and desired work location. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines including security staff is critical. • Provides psychiatric care, including admission evaluations, follow-ups and urgent and emergent consultations to individuals enrolled in the mental health service, • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication, • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics, • Regularly participate in the creation of individual treatment plans for patients, and review plans already in place, • Evaluate patients for the need for a higher level of care, including hospitalizations. Position Overview Under supervision of the Site Medical Director, the Physician will provide comprehensive, compassionate, and thoughtful care to patients with complex chronic disease in the New York City jail system. The Physician will be part of a core interdisciplinary team working in a unique environment delivering the care to patients with significant chronic illnesses. The Physician will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services. The interdisciplinary team will work under supervision of a Site Medical Director. Responsibilities include: • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated., • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable., • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the medical leadership and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Collaborate closely with CHS Physician Assistants, including providing clinical guidance, cosigning notes, and providing other supervision based on clinical circumstance and PA requirements., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the appropriate parties, including Urgicare, about 3-hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. State Inspections: Ambulances are subject to inspection by representatives of the State EMS office without advance notice. Inspectors will show proper identification and conduct inspections on the spot. In the event of an inspection, the crew must notify the dispatcher immediately, then cooperate fully with the inspector. As requested by the inspector, the crew will present or demonstrate any or all equipment on the ambulance. At the completion of the inspection, the inspector will present the crew with a copy of the inspection sheet that will be turned over to the supervisor at the end of the shift. The crew will promptly notify the dispatcher of the results of the inspection. In the event a violation notice is issued, the crew must contact a supervisor prior to accepting another patient. In addition to the Radiology Technologist job functions, the QA Technologists is responsible for the following: • Organize and file maintenance reports for each building in regards to dentistry and radiology x-ray equipment and ensure dentistry and x-ray equipment follow the Department of Health codes, policies and procedures., • Participates in quality assurance activities, including coordination, control and maintenance of technical equipment., • Establish a daily/weekly/monthly quality control routine for monitoring, evaluating, and maintenance of X-ray equipment to ensure optimal performance and stability., • Documenting and maintaining records for the quality control program in accordance with applicable regulations, legal requirements, accrediting agencies and recommendations from equipment manufacturers., • Performs research and participates in special projects involving evaluation of equipment and delivery of service., • Work with our radiologists to obtain updated documentation of primary diagnostic monitors., • Distribute/collect dosimeter badges from each employee/facility that works with x-ray equipment., • Recommends area assignment of personnel to coordinate activities within the department with other activities and services and represents the department in interdepartmental operations and meetings., • May be assigned duties and responsibilities of X-ray Director in regards to radiology department workflow. Minimum Qualifications Assignment Level III 1. For Supervisory Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and two (2) years of Radiologic Technologist experience performing duties consistent with Assignment Levels I and II, one (1) year of which must have been in a supervisory capacity., 2. For Specialized Imaging Assignments: a valid license and current registration to practice Diagnostic Radiography as a Radiologic Technologist issued by the NYSDOH; and possession of a current advanced- level certification for the performance of these procedures.
