Job Description JOB SUMMARY Under the primary supervision of the Clinical Supervisor, and/or Clinical Director of Integrated Services, the Primary Counselor functions as part of the treatment team and provides comprehensive treatment services including substance use disorder counseling services, client assessment, screenings, case management, care coordination, individual counseling services, group counseling, psychoeducation classes, treatment planning, continuum of care referrals, and crisis intervention, etc., to clients struggling with substance use disorder. Job Description WEST SIDE FEDERATION FOR SENIOR AND SUPPORTIVE HOUSING Social Worker I (Bilingual /Spanish) The Social Worker I works as part of the social service team. They report to the Clinical Coordinator, or Director of Clinical and Social Services, while working closely with their Building Manager. A Social Worker I is an exempt employee who is required to have a Masters’ Degree. They are expected to be able to work autonomously while developing 1:1 relationships with the residents. This relationship will enable the Social Worker I to provide a comprehensive range of social work services to the residents and enable the residents to establish and maintain safe and independent lifestyles in the community. WSFSSH strongly supports M.S.W. level social workers obtaining their licensing. We offer study groups for staff who are preparing to take their exam. WSFSSH will also pay for a study course if needed, and pays for up to 2 licensing exams, as well as registration/licensing fees. Once a MSW staff member has obtained their LMSW, WSFSSH will cover reasonably priced ceu credits in order to meet licensing requirements. In addition, WSFSSH will provide weekly clinical supervision by a LCSW, and because WSFSSH is a waivered agency with the NYS Education Dept, a LMSW's work experience will count towards their LCSW. Job Description JOB SUMMARY Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0CV-4165 (NGG) O’Toole et al. v. This settlement requires the state to assist residents of impacted adult homes identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap-around services are in place after the move. An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. The Social Worker will be responsible for conducting independent clinical assessments, providing referrals to community-based services, offering comprehensive clinical case management, and monitoring participant compliance. They report to the Clinical Coordinator, or Director of Clinical and Social Services, while working closely with their Building Manager. Job Description THE ORGANIZATION The Center for Justice Innovation is a nonprofit organization dedicated to advancing community safety and racial justice. Since 1996, we’ve worked alongside communities, courts, and those most directly affected by the justice system to build stronger, healthier, and more equitable neighborhoods. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY The Brooklyn Mental Health Court (BMHC) launched in 2002 as NYC’s first mental health court, serving individuals charged primarily with felony offenses who are living with serious mental illness. The program diverts participants from jail or prison by conducting clinical assessments and then connecting them with long-term treatment in the community, along with rigorous judicial monitoring. The court aims to strengthen the justice system’s ability to identify, assess, and monitor individual participants, to create linkages between the justice and mental health systems, and to improve public safety by ensuring that participants receive high quality community-based services. The Brooklyn Felony ATI Program utilizes a similar model to Brooklyn Mental Health Court, serving individuals facing felony charges who are not otherwise eligible for Mental Health Court or other treatment courts in Brooklyn. The program receives referrals through two pathways: (1) the dedicated Felony ATI court part within Brooklyn Supreme Court, with a dedicated judge and supervising prosecutor; and (2) through other court parts throughout Brooklyn Criminal and Supreme Court. Regardless of the referral pathway, all participants receive individualized assessments, onsite and community-based services, compliance monitoring and judicial monitoring for the life of the case. BMHC is seeking a Social Worker for its Brooklyn Felony Diversion programs. The Social Worker will serve individuals who are mandated to Felony Alternative to Incarceration (Felony ATI) programming through the Brooklyn Supreme Court. Reporting to the Clinical Director, the Social Worker is responsible for conducting psychosocial assessments, linking clients to a range of wrap-around services, providing comprehensive trauma-informed case management, compliance monitoring, and reporting for clients referred to the Brooklyn Mental Health Court and Brooklyn Felony ATI programs.
