Our Heart Failure program is recognized for high-quality, comprehensive care and provides Advanced Cardiac Therapies to our region. We completed Joint Commission Certification for Destination TherapyLVAD care in 2018 and have a dedicated inpatient service and office space to manage the full spectrum of heart failure patients. Job Summary This position is responsible for the management of properties serving formerly homeless and very low-income disabled individuals with multiple barriers to housing stability. Assure adherence to community policies, implement policy-based sanctions up to and including evictions. * Create notices for non-compliance actions for supervisor approval. * Assure that the cleanliness of common areas is maintained daily, landscaping is in good condition, and minor repairs are completed. Provide oversight of timely renovation of vacant apartments. * Utilize maintenance software programs to enter and track work orders and regularly review maintenance reports. Keep all records as required and provide corporate reports. * Monitors and documents security events, maintenance issues and problematic visitor and resident behavior. Staff Relations: * Establishes and maintains regular daily office hours, ensuring adequate coverage. * Maintain positive communication daily with service staff if applicable on resident issues to assure seamless, comprehensive, wrap-around services, as well as seeking opportunities to integrate skills sets with OSHS/Housing Social Services/Activities Coordinator, if applicable. * Meet regularly with Maintenance staff offering support needed to carry out responsibilities in a competent, professional manner. * Conducts health and safety quality control inspections to detect possible violations of housing quality standards and workplace safety/security hazards. Property Program Duties: * Screens, reviews, and approves all applications for submission to corporate. * Responsible for delivery of recertification notices to tenants in accordance with Program specific requirements. * Follow up on annual re-certifications/renewals after notifications have been sent. * Schedule and perform certification appointments with applicants and residents. * Inspect apartments for move-in condition (pre-inspection) and turnover status. * Prepare move-in files for new residents. * Assist Compliance Department by performing peer review file audits of MI, IR, AR, IC, TM, corrections and full tenant files. * Assist Compliance Department in the resolution of discrepancies from certification corrections, MOR, EIV and TRACS; and documentation of the resolution in accordance with HUD requirements as requested by Sr. PM. * Assist with the preparations for the annual audits and inspections as requested by Sr. PM. . * Assist in the maintenance of affordable housing waitlists in accordance with Program specific requirements. * Assure compliance with regulatory requirements, including Fair Housing guidelines, recordkeeping, reporting and audits. Report accidents and emergency situations to the Sr. Property Manager immediately and prepare Incident reports. * Ensure that all maintenance requests are handled in a timely manner and that residents are notified if parts must be ordered, causing a delay. Enforce weekly follow-up of such requests. Additional Duties: * Make daily bank deposits and maintain petty cash fund monthly. * Review all bills and invoices for payment after matching approvals. * Review POs, receive them timely. * Participates in the preparation of the annual operating budget and works with Sr. Property Manager to maintain budgetary guidelines. * Keep all records as required and provide monthly reports including dashboards, delinquencies, unit turnover and extended vacancies. * Perform monthly property inspections with the Regional Managers to ensure that vacant units are filled, and the property is kept up to professional standards. * Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure theconfidentiality,integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA). * Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor. * As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect. * Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine. * Perform related functions necessary to support the mission and core values of Catholic Charities Job Qualifications To perform this job successfully, an individual must meet the minimum qualifications listed below. Responsibilities • Complete all Onboarding requirements within 2 weeks of start date, • Check-in with your assigned Charlie Health Group Quality Supervisor or Group Quality Director at a minimum of 1x/month, • Respond to all email and Slack communication promptly (within 48 hours), • Review the curriculum aligned to group assignment prior to group start time, • Arrive ~10 minutes early to scheduled group time and facilitate all groups for the entirety of the hour, • Facilitate groups using the current Charlie Health curriculum and best practices, • Facilitate groups across age groups and cohorts, including Integrative curriculum, Support Staffing and Wellness Hour as needed, • Foster client engagement and group cohesion, encouraging client participation and fostering camera-on culture, • Following all operational policies and procedures as indicated by Charlie Health best practices, • Participate in collaborative Therapy Treatment Team (Tuesdays) and Group Supervision (every other Friday) with your assigned Charlie Health Group Quality Supervisor or Group Quality Director, peers and other Clinical Leadership team members, • Collaborate closely with Primary Therapists, Care Experience Specialists and Care Coaches, • Communicate professionally and promptly with all clients, staff, families, agencies, and referents, • Monitor your treatment documentation to ensure all of your notes are completed within required 24 hour timeframe and meet agency and professional standards per DHCS and The Joint Commission standards, • Demonstrate professional ethics, including appropriate boundaries and confidentiality, • Availability to work weeknights (3-8pm MT on Monday to Thursday) and Saturdays, • Master’s degree in mental health or related field (see examples below), • Experience working with diverse age demographics in intensive treatment settings, • Demonstrated proficiency across multiple treatment modalities, with the confidence to thoughtfully integrate them into clinical practice (e.g., DBT, CBT, EMDR; MI certification a plus), • Strong belief in and advocacy for group-based treatment alongside individual therapy, • Ability to facilitate effective, engaging telehealth sessions, • Reliable high-speed internet connection for client sessions, • Proficiency with cloud-based communication software (Gmail, Slack, Zoom, Dropbox) as well as EMR and outcomes survey software, • Master of Science (M.S.) in Mental Health Counseling, • Master of Social Work (M.S.W.), • Master of Arts (M.A.) in Marriage and Family Therapy, • Master of Arts (M.A.) in Clinical Psychology Charlie Health is pleased to offer comprehensive benefits to all full-time, exempt employees.
