Job summary: Experienced Peer Specialist to work on a newly funded innovative Care Transition team that serves participants transitioning to the community from a higher level of care. As a member of the Pathway Home multi-disciplinary team the Peer Specialist position will work under the supervision of the Team Leader. The role will involve facilitating transition from one level of care to the community by addressing the preparatory needs of participants in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through team and individual work in the community. The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Peer Specialist engages participants in peer-based services to identify and overcome barriers to community integration, utilizing a peer relationship to foster connection, trust, understanding and validation with community providers and supports. The role will require on call coverage and a willingness to work flexible hours. Job Responsibilities: • Engagement beginning from referring location with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the participant;, • Individual sessions with participants emphasizing prevention and preparing for independent community living and the promotion of optimum mental and emotional health. May require helping participants deal with issues associated with but not limited to family and social relationships; stress and symptom management; activities of daily living; medication management; and housing readiness;, • Provide intensive emotional and practical support to participants as they transition back into their communities and into a supportive housing and/or living environment;, • Work with participants and their housing providers to resolve clinical issues that are impacting on the participant’s ability to obtain, manage, and retain supportive housing;, • Assist and instruct participants in attending to daily living skills such as personal hygiene, grooming and laundry, nutrition/meal preparation, budgeting, and socialization tasks and skills needed to successfully live and work in the community environments of their choice and also in maintaining a safe living environment;, • Foster and maintain cooperative and collaborative relationships with community providers and network members to facilitate connections or re-connections to services and supports;, • Community navigation including accompanying to first behavioral health and medical appointment, travel training, reengagement in community care, referral to services with ability to identify and address potential services and barriers to obtain and maintain such services;, • Establish collaborative working relationships with referring treatment teams and other partners and plan with them for appropriate discharges for participants;, • Advocate with and for program participants to achieve personal goals and address barriers such as fear, stigma and communication disparities with community providers and supports;, • Provide on-call after hour crisis intervention services when needed to participants and their support network, including respite referrals and other diversion and stabilization services;, • Attend and participate in team meetings and supervisory sessions;, • Develop short-term person-centered treatment plans to assist participant towards achieving their goals;, • Monitor and record participant’s progress and modify plan according to needs and preference;, • Perform various needs assessment for essential ongoing community integration and support;, • Timely and accurate documentation of participant information within Health Information Technology platform, • Comply with all required in-service training and staff development;, • Perform other related duties as assigned; Essential Knowledge, Skills and Abilities: · Knowledge of Behavioral Health systems, Mental health legislation and regulations, and health conditions ·Ability to manage multiple projects and ask for help when needed ·Ability to participate on a team to accomplish tasks ·Serves as a role model to staff and stakeholders ·Ability to work with all stakeholders: participants, network, families, and government staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences ·Knowledge of treatment, rehabilitation, and community support programs as they relate to members and their families ·Knowledge of techniques for identifying risk, including crisis management techniques Minimum Education and Experience Requirements: Minimum High School Diploma or equivalent or higher. Van Leeuwen Ice Cream is hiring a full-time Logistics & Inventory Coordinator to join our Supply Chain team. As we continue to scale our CPG business across retail channels nationwide, this role is critical to ensuring our products move efficiently, on time, and on budget from our production facility to distributors, retail partners, and DTC fulfillment centers. Beyond freight, this role owns CPG inventory management end to end, weeks of supply (WOS) tracking, on-time-in-full (OTIF) performance at retailers, and inventory transfers across our 3PL network and supports our scoop shop business through 3PL inventory management, intercompany freight coordination, and logistics cost management and controls. This is a high-impact, cross-functional role sitting at the intersection of operations, vendor management, and customer service. You'll work closely with our production, sales, and finance teams to coordinate domestic freight movements, manage carrier relationships, manage inventory health across our distributor and 3PL network, and help build the logistics infrastructure that supports Van Leeuwen's next stage of growth. This role will be based out of our Bronx office and follow Van Leeuwen's Hybrid work model and be in office at least two to three days a week. Job Responsibilities • CPG Inventory Management, • Own CPG finished goods inventory visibility and accuracy across the network — production facility, 3PL warehouses, distributors, and DTC fulfillment centers, • Manage weeks of supply (WOS) by SKU and location at distributors and retailers, monitoring sell-through and flagging out-of-stock and overstock risks before they impact service or margin, • Plan and execute inventory transfers between 3PL warehouses, distributors, and fulfillment centers to balance stock, meet demand, and minimize freight and storage costs, • Reconcile 3PL and WMS inventory records against the ERP, investigating discrepancies, shrink, and damaged or short-dated product and driving root-cause resolution, • Report inventory KPIs (WOS, fill rate, inventory accuracy, aging) on a weekly cadence with clear recommendations for rebalancing or replenishment, • Freight & Transportation Management, • Coordinate inbound and outbound freight movements across LTL, FTL, and refrigerated transport for CPG retail and distributor accounts, • Book and manage shipments with carriers and 3PL partners, ensuring on-time pickup and delivery, • Track shipments in real time and proactively communicate delays or disruptions to internal stakeholders and retail partners, • Manage freight claims from initiation to resolution, including documentation and carrier follow-up, • Ensure all shipments meet temperature and cold chain compliance standards required for frozen CPG products, • Distributor & Retail Compliance, • Coordinate order fulfillment for key retail accounts (grocery, natural, club, and specialty channels), ensuring compliance with retailer routing guides and vendor requirements, • Manage ASN creation and EDI coordination with retail partners and distributors, • Monitor and reduce chargebacks and deductions related to logistics non-compliance, • Manage OTIF (On-Time, In-Full) performance at retail partners — monitor retailer scorecards, perform root-cause analysis on misses, and implement corrective actions to minimize fines and protect vendor standing, • Maintain up-to-date knowledge of retailer-specific routing, labeling, and scheduling requirements, • Vendor & Carrier Relations, • Build and maintain strong relationships with carriers, brokers, and 3PL partners, • Solicit and evaluate freight quotes, supporting cost optimization across the supply chain, • Assist in identifying and onboarding new logistics partners as the network scales, • Scoop Shop Supply Chain Support, • Manage 3PL inventory and weeks of supply (WOS) for the scoop shop business, ensuring product availability for shops while minimizing excess, aged, and short-dated stock, • Coordinate intercompany freight between production facilities, 3PL warehouses, and scoop shop distribution points, ensuring on-time, temperature-compliant, and cost-effective deliveries, • Own cost management and controls for scoop shop logistics — track freight and storage spend against budget, audit carrier and 3PL invoices, flag variances, and identify savings opportunities, • Support intercompany inventory transfers and reconciliation between the CPG and scoop shop businesses, ensuring accurate intercompany records in the ERP, • Operations & Reporting, • Maintain accurate records of all shipments, freight costs, and carrier performance in TMS and ERP systems, • Generate regular freight spend and performance reports, identifying trends and cost-saving opportunities, • Collaborate with production and inventory planning teams to align shipment scheduling with production output and inventory availability, • Support month-end close by reconciling freight invoices and accruals Job Requirements • 2 to 4 years of experience in logistics, freight coordination, inventory management, or supply chain, preferably within CPG, food & beverage, or perishable goods, • Bachelor's degree in Supply Chain, Logistics, Business, or related field preferred, • Familiarity with LTL, FTL, and refrigerated freight operations, • Experience working with distributors and retail accounts (UNFI, KeHE, DPI, or similar a plus), • Working knowledge of routing guides, EDI, and ASN processes, • Proficiency in TMS, ERP, or WMS platforms; NetSuite or SAP experience a plus, • Hands-on experience managing inventory KPIs: WOS, OTIF, fill rate, and inventory accuracy, including root-cause analysis and corrective action follow-through, • Experience managing inventory at third-party (3PL) warehouses, including transfers, reconciliations, and cycle count support, • Strong Excel / Google Sheets skills (pivot tables, lookups) for inventory, freight, and cost analysis and reporting, • Familiarity with freight budgeting, invoice auditing, and cost controls; The Department of Psychiatry is seeking psychologists assigned to work in the CUIMC FPO. These are full-time, clinical positions in the Columbia Day Program. Primary responsibilities involve conducting group therapy utilizing empirically supported treatments, diagnostic/intake evaluations of adults, and individual treatment planning and progress monitoring with patients. Candidates should have previous experience providing outpatient treatment and working with a multi-disciplinary team. Experience and training in Dialectical Behavior Therapy (DBT) and Cognitive Behavioral Therapy (CBT) preferred. Candidates with additional experience in evidence-supported treatment delivery for trauma and/or substance use are desired. Ideal applicants will have experience, familiarity and comfort working with high-risk patients, as well as young adults and college students. Applicants should have strong interpersonal skills and the ability to work well across disciplines and function effectively as part of a multi disciplinary team.
