This position offers competitive pay and the opportunity to provide skilled nursing care to patients in their place of residence while coordinating with an interdisciplinary care team. This is a great opportunity for experienced home health RNs who are proficient in Homecare Homebase (HCHB) and OASIS and can support local travel and on-call needs as required. RN Home Health Compensation • Pay Rate: $57.23/hrSchedule, • Shift: Full-time, 40 hrs/week (flexible schedule), • On-call: may be required based on staff/branch needs (subject to change), • Expected weekly productivity: 30 units, • Local travel required, • Float required: YesAdditional Information, • On-site / field-based role (patient care in the home), • Must have reliable transportation, insurance, and a valid driver’s license, • Must be proficient in HCHB and experienced with OASIS, • Strong documentation and communication required to ensure continuity of care and payer complianceRN Home Health Responsibilities, • Administer skilled nursing care for clients of all ages in their place of residence, • Coordinate care with the interdisciplinary team, patient/family, and referring agency, • Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs, • Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity, • Maintain up-to-date clinical records reflecting visits, contacts, problems, actions, goals, and changes in condition, • Provide care utilizing infection control measures that protect staff and patients (OSHA), • Educate patients/families regarding care, diet, medications, home safety, and self-care practices, • Supervise and provide clinical direction to support staff (HHA/STNA/LVN) to ensure quality service, • Monitor assigned cases to ensure compliance with third-party payer requirements, • Communicate effectively and serve as a liaison with patients/families, physicians, and care team members, • Perform other related duties as assigned or requestedRN Home Health Requirements, • Current state RN license (required), • Graduate of an accredited ADN program or Nursing Diploma (required), • 1–2 years RN experience in a clinical care setting (required), • 2+ years home health experience (required), • Homecare Homebase (HCHB) experience (required), • OASIS experience (required), • Proof of current CPR certification (required), • Current driver’s license (required), • Reliable transportation and insurance (required), • Knowledge of medications and correct administration; competency with venipuncture/IV care, • Ability to work independently, prioritize tasks, and function under stressPreferred Qualifications, • BSN (preferred), • CHPN certification (preferred)Benefits, • Benefits available to full-time employees after 90 days of employment Since 2004, we’ve specialized in placing talented healthcare professionals in roles where they can make a real impact -whether that’s in hospitals, clinics, health plans, or community-based care settings.
