Seeking a Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients in Fayetteville, North Carolina. Details: • Full-time, Mon - Fri, 8am-5pm, • 13-16 patients per day, • Geriatric population, • Provision of exceptional primary care., • Conduct office visits for routine and acute issues., • Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation., • Care coordination with other providers, specialists, testing facilities, and agencies., • Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years), • Assisting the care team with phone triage and outreach., • Conducting home visits as needed Qualifications: • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required, • National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC, • Experience in primary care: Internal medicine, geriatrics, or family medicine preferred, • Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
