Job DescriptionDescription: Purpose of Position: The Lead Medical Coordinator - Registered Nurse (RN) is responsible for overseeing health services as well as coordinating the provision of timely, appropriate health services for children in care. They supervise other Medical Coordinators and provide appropriate staff training on medical, dental, and public health topics relevant to pediatric, adolescent, and migrant health. They are responsible for developing and maintaining policies and procedures for confidential health record management, triage of health concerns, medical emergency response, environmental health and safety, and post-release health care planning/care coordination. In addition, this role is critical in assessing patient needs, developing and implementing individualized care plans, and ensuring the highest standards of safety and quality in a dynamic clinical environment. The RN serves as a primary advocate for patients, providing education and support while maintaining meticulous documentation and adhering to all regulatory and organizational policies. Pay Range: $40.71 - $58.15 per hour. Requirements: DESCRIPTION OF DUTIES: Patient Care and Clinical Management: • Conduct comprehensive assessments of patient health status, including physical, psychological, and social factors., • Develop, implement, and evaluate individualized patient care plans based on assessments, physician orders, and patient needs., • Administer medications safely and accurately, adhering to the "five rights" of medication administration and organizational policies., • Perform various medical treatments, procedures, and interventions with precision and professionalism., • Monitor patient conditions, interpret diagnostic data, and respond effectively to changes in patient status., • Collaborate with physicians, therapists, pharmacists, and other healthcare professionals to ensure coordinated and holistic patient care., • Serve as a primary point of contact for patients and their families, providing updates and emotional support., • Supervise the Medical Department staff and others assigned as deemed necessary and adjunct functions, ensuring efficient and effective workflow providing guidance, support and professional development opportunities, and performance management of team members., • Develop and implement policies and procedures that promote the organization's mission and values., • Monitor and evaluate departmental performance, identifying areas for improvement and implementing corrective actions as needed., • Maintain accurate, timely, and thorough patient records in electronic health systems (EHR)., • Ensure all nursing practices and documentation comply with company policies, legal requirements, and regulatory standards., • Communicate critical patient information effectively to the healthcare team, promoting seamless transitions of care., • Educate patients and their families on diagnoses, treatment plans, medication management, and self-care techniques., • Advocate for patient rights, preferences, and needs, ensuring their active participation in their healthcare journey., • Bachelor of Science in Nursing (BSN) from an accredited program., • Current, unencumbered Registered Nurse (RN) license in the state of California. Strong combination of clinical expertise, critical thinking, and interpersonal skills with additional foci on detail-oriented actions and the ability to manage multiple priorities in a fast-paced, high-pressure environment.
