The Patient Services Specialist serves as the primary point of contact between patients, families, healthcare providers, and the organization. This role ensures that patients receive high-quality, coordinated care by facilitating communication, managing appointments, addressing concerns, and providing education and support throughout the care process. The Patient Care Coordinator/Liaison advocates for patient needs and works to enhance the overall patient experience. Key Responsibilities: • Serve as the main contact for patients and families, addressing questions, concerns, and requests promptly and professionally., • Coordinate patient care activities, including scheduling appointments, procedures, and follow-up visits., • Facilitate communication between patients, families, physicians, nurses, and other members of the healthcare team., • Assist patients in understanding care plans, treatment options, insurance benefits, and available resources., • Monitor patient progress and follow up to ensure continuity of care., • Support patient transitions between hospital, outpatient, and home settings., • Educate patients and families on medical processes, post-care instructions, and community resources., • Advocate for patient needs to ensure culturally competent, patient-centered care., • Maintain accurate and confidential patient records and documentation., • Identify and address barriers to care, such as transportation, financial assistance, or language needs. Qualifications: • Bachelor’s degree in healthcare administration, nursing, social work, or a related field (preferred; equivalent experience may be considered)., • Experience in patient care coordination, case management, or a healthcare-related field., • Strong interpersonal, communication, and conflict-resolution skills., • Knowledge of healthcare systems, insurance processes, and community resources., • Proficiency with electronic health records (EHR) and Microsoft Office Suite., • Ability to work independently while collaborating with multidisciplinary teams., • Compassionate, patient-centered approach with strong organizational skills. Work Environment: • Remote with 25-40% travel to healthcare settings (hospital, clinic, or outpatient facility)., • Fast-paced environment requiring adaptability and multitasking.
