Job Description Connected Cardiovascular Care Associates (C3) is seeking an experienced Nurse Practitioner or Physician Assistant to join our growing cardiology practice serving the Heath and Dallas communities. This role offers a balanced mix of outpatient clinic care and light inpatient rounding, within a collaborative, physician-led environment. About Connected Cardiovascular Care Associates (C3) C3 is an innovative, physician-led cardiovascular practice founded by Dr. Tony Das, a nationally recognized interventional cardiologist. Our practice delivers comprehensive cardiovascular care across coronary, vascular, structural heart, electrophysiology, heart failure, and preventive cardiology. Our physician team includes: • Dr. Imran Baig – Interventional & Structural Cardiology, • Dr. Nicholas C. Macpherson – Cardiovascular Imaging & Heart Failure, • Dr. Azizul Rehman – Interventional Cardiology, • Dr. Ken Johnson – Interventional Cardiology, • Dr. Syed Saad – Electrophysiology Learn more: Position Overview • Schedule: Monday–Thursday 8:30 AM – 5:00 PM, Friday 8:30 AM – 3:00 PM, • Locations: Weekly rotation between Dallas and Heath clinics, • Hospital Rounding: Light inpatient rounding across Dallas, Plano, Rockwall, and Rowlett, • Call: 1:4 weekend rotation Key Responsibilities • Evaluate, diagnose, and manage cardiology patients in the outpatient setting, • Perform and document history, physical exams, and follow-up care in the EMR, • Review and communicate diagnostic testing results, including imaging, • Collaborate with physicians, APPs, and clinical staff on patient care plans, • Triage patient needs, including messaging, pre-operative clearance, and peer-to-peer reviews, • NP or PA from an accredited program, • Nurse Practitioners: AGACNP required, • Minimum 5 years of cardiology experience required, • Active Texas license in good standing, • DEA and Controlled Substance Registration, • Physician-led, clinically focused environment, • Strong support staff and established workflows, • Access to advanced cardiovascular imaging and technology, • Balanced schedule designed to support long-term sustainability, • Competitive base salary (commensurate with experience), • Productivity-based bonus structure, • Malpractice coverage, • CME allowance, • 401(k) with immediate vesting and employer match, • Medical, dental, and vision insurance, • Paid time off, • Disability and life insurance 99 Healthcare Management is a Dallas-based physician practice management company.
