The LVN will also assume the role of patient advocate and liaison between the care team and patient. ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following functions: * Assisting with daily personal tasks that include basic personal hygiene and grooming, feeding, and ambulation. * Assisting with household activities such as housekeeping, laundry, bed making, and cleaning. * Aiding in meal preparations and making sure that their dietary preferences andrestrictions are observed. * Arranging transportation and accompanying clients to their medical appointment, visiting any family/friends, or any recreational activities to encourage social engagement. * Documenting and keeping track of all medical and dental appointments, including the administration of medication. * Ensuring the safety and well-being of the clients by maintaining a clean and healthy environment. * Monitoring and reporting any changes in the behavior, health, or emotional well-being of the clients. * Maintain a respectful and supportive attitude towards the individuals being served. * Participate in on-the-job training sessions to enhance skills and knowledge. * Performs other related duties as assigned.
