The Clinical Pharmacist will collaborate with primary care providers, clinic team members, and pharmacy team members to provide a cohesive care team to benefit the health of patients. The Clinical Pharmacist participates in day-to-day clinical pharmacy operations through collaborative practice agreements. For clinical operations, activities may include, but are not limited to: patient counseling, consultations with provider and nursing teams, treatment adherence reporting and drug monitoring, coordination of long acting injectables, and assisting the prescribers in the appropriate ordering of pharmaceuticals and supplies. This is a 13-week contract position. Schedule-D/E/N Rotating Shift Schedule - Weekends, Holidays/On-Call/Callback/Charge per Unit Needs 36 or 48 hours per week. The neurosurgical unit offers extraordinary opportunities to care for a diverse patient population. Work with multidisciplinary teams specializing in brain tumors, spinal malformations, neuroendovascular, and epilepsy surgery. This is a mixed-acuity unit with both floor-level and an 8-bed high-acuity step-down unit for patients with more complex needs who require hemodynamic and neurological monitoring. Provide high-quality nursing care to patients in a neurosurgical unit. • Conduct Q1 neuro checks for neurological assessment., • Manage EVDs, lumbar drains, and subdural drains., • Administer 3% NS, Cardene drips, and arterial lines., • Assist in carotid endarterectomy, aneurysm coiling/clips, and DBS insertion (Deep Brain Stimulator)., • Manage VP shunts and seizure management as required. Must have stepdown/PCU experience. • Two (2) years of nursing experience in high acuity stroke and neurosurgery., • BLS certification (American Heart Association ONLY)., • Telemetry experience required., • NIHSS certification required., • RN License (Only NY Licenses are accepted at this time)., • ACLS certification preferred. As a TSH Navigator, you’ll be a vital member of our care team; a trusted partner and advocate for a dedicated panel of patients throughout their healthcare journey. You’ll go beyond the standard duties of a medical assistant to build real relationships, learn each patient’s individual needs, and ensure their care is well-coordinated and genuinely personal. You will be the primary point of contact for your patients–whether in the clinic, or between visits. Patient Care & Clinical Support • Greet, check in, and room patients; update demographic, pharmacy, and billing information, • Take and record vital signs, administer visit-appropriate screening tools, document reason for visit and current symptoms, and prepare patients for exams, • Conduct laboratory procedures including point-of-care testing, specimen collection, and processing Care Coordination & Navigation • Serve as the primary point of contact for a dedicated patient panel, • Communicate with patients to support follow-up, answer questions, and coordinate care, • Coordinate referrals, specialty appointments, and medical records; support patients in addressing social determinants of health Who You Are You’re a natural connector.
