In this role, you will report to the Senior Manager of the Technical Operations Centre and play a key part in supporting and maintaining complex, cutting-edge broadcast video systems. Job Description SUMMARY Renders comprehensive health care within the limitations of professional licensing to patients of the Los Angeles Christian Health Centers. Assesses needs and prescribes treatment. Compassionately and competently provides primary and preventive health care to all patients, including patients with acute and chronic illnesses, substance use disorders, mental illness, and homelessness. Provides clinical supervision to Advanced Practice Providers working on our provider team. Assists the CMO in overseeing clinical services throughout LACHC sites. The clinic requirement is to lock all progress notes within 48 hours., • Review and electronically sign off on all laboratory results, diagnostic results, and outside consult notes in a timely manner, ensuring patient is contacted for follow up if clinically appropriate., • Initiate and follow through on specialist referrals and e-consults (electronic consults), communicating with specialist providers as necessary, in the provision of whole- person care., • Serve as the primary care physician for a panel of patients, working with a care team and other staff at LACHC to serve as a patient- centered medical home for our patients., • Clinically supervise Advanced Practice Providers (APPs) employed by LACHC., • This includes thoroughly reviewing a minimum of 50 charts of all new providers under their clinical supervision, per guidelines for supervision of new providers to confirm clinical competence, and reporting findings to the Chief Medical Officer., • This includes ongoing clinical supervision of designated APPs and as-needed clinical supervision of other APPs., • Conjointly supervise medical assistants to improve care team efficiency and efficacy, along with Medical Assistant Supervisor., • Participate in LACHC meetings including morning “team huddles”, Provider Meetings, and All-Staff Meetings., • Provide compassionate, competent care for patients who, depending on the clinic site, may experience homelessness, may suffer from mental illness or substance use disorder, and often lack resources to care well for themselves., • If seeing adult patients, may be asked to complete waiver training to provide Medication Assisted Treatment (MAT) in order to care for patients with substance use disorder., • Actively participate in Continuous Quality Improvement activities at LACHC, including teaching other providers on selected topics, implementing team- based care, participating in peer reviews, evaluating clinical performance measures, and implementing improvements using a Plan-Do-Study-Act method of stepwise changes., • Periodically serve “on call” for the clinic, answering urgent phone calls from patients or other agencies in the off-hours for one week approximately every 10 weeks., • Must be available to periodically work a Sunday shift as required to support organizational operations, • Network with other health care agencies and county facilities as needed or requested., • Assist in provision of health screenings (including TB, diabetes, hypertension) at outreach events., • Maintain active enrollment in Medicare, Medicaid, Managed Care, and other payor programs as needed for clinical operations, completing credentialing paperwork in a timely manner to remain actively enrolled in these programs., • Other duties may be assigned. Job Description Position Description: The Behavioral Health Peer Support Navigator is embedded in the KP LAMC Emergency department and engages homeless patients and high utilizers of Emergency Department services. The Peer Support Navigator provides education on the benefits of being connected to medical homes, such as Wesley, informs them of the services offered through our network of clinics, and assists these patients in scheduling an appointment and supports linkage to Wesley Health Centers and other services as appropriate. This position also works in concert with the Patient Navigator to help prevent insufficient coverage during periods of high volume. The Behavioral Health Peer Support Navigators provide recovery-oriented, culturally appropriate services that promotes engagement, socialization, self-sufficiency, self-advocacy, natural supports and are trauma aware. The Peer Support Navigator conducts initial brief assessments, introduce patients to MOUD programs & services, and expedite appointments at MOUD-capable clinics. They also assist patients in accessing additional services such as financial counseling, primary care, mental health services, case management, housing support, social services, and residential treatment programs. Note: This position is stationed within Kaiser Sunset Emergency Department and candidate will be subject to policies of both JWCH institute and Kaiser Permanente. This position will may be required to work Saturdays and potentially some evenings. SUMMARY Provides primary care medical services to the patients of UMMA. ESSENTIAL DUTIES • Provides thorough, compassionate, evidence-based patient care, • Reviews patient record, including allergies, problems, medications and immunization status., • Elicits and records patient’s medical history and social determinants of health., • Accurate and thorough SOAP note documentation., • Examines patient to determine general physical condition., • Orders and accurately interprets only necessary laboratory tests, pathology results, radiologist reports in the development and execution of a treatment plan for patients., • Documents gathered information in medical record as required by policies and analyses reports and findings of tests and examinations., • Prescribes treatments and medication utilizing evidence-based medicine and based on patient’s condition and allergies., • Discusses any possible side effects to medication or immunization with patient., • Prescribes vaccinations to immunize patient from communicable diseases., • Advises patients concerning diet, hygiene, and methods for primary and secondary prevention of disease., • May provide prenatal care to pregnant women and may provide postnatal care to mother and infant, if applicable., • Refers patients to medical specialist or other practitioner for specialized treatment., • May perform minor surgery/procedures based on privileges granted., • Attempt to address all care gaps at each visit., • Follows up with patient regarding progress in high risk or emergency cases., • Conducts physical examinations to provide information needed for admission to school, consideration for jobs, or eligibility for insurance coverage., • Administer Family Planning services in compliance with title X Rules and Regulations/Protocols., • Ability to develop positive rapport with patients and families to foster the provider/patient relationship., • Active participation in the Quality Improvement Program, Peer Review, Provider and clinical meetings, as well as UMMA’s Coverage and On-call schedule., • Provide preventative health care services in accordance with HEDIS and other national evidence based standards., • Able to complete patient charting in a timely fashion to support billing levels., • Able to address all inbox messages in a timely manner., • Maintain current knowledge-base and appropriate licensure, • Able to work 1 Saturday a month if need is identified, • Able to work collaboratively as a Care Team member, • Able to supervise NPs and Pas, • Able to precept medical studentsSECONDARY DUTIES, • Supports UMMA fundraising objectives., • Provides leadership support to clinical staff members.
