Job DescriptionStarting at $85,100 MAJOR FUNCTION The Clinical Pharmacist works in collaboration with HJA providers to provide top quality care to its patients and the general community. Clinical Pharmacist will provide direct clinical care to patients including medication therapy management, participation in collaborative practice agreements, coordination of treatment with licensed independent practitioners, and general patient care management. The Clinical Pharmacist will support the Lead Clinical Pharmacist in carrying out new and established clinical pharmacy initiatives to improve patient health outcomes and clinical pharmacy services reimbursement. ESSENTIAL FUNCTIONS • In collaboration with the CMO and HJA clinical staff, designs, develops and implements protocols and criterion to determine patients requiring close medication monitoring.
