ESSENTIAL JOB FUNCTIONS AND ACCOUNTABILITIES Population Health Analytics • Conduct analyses of clinical, claims, and administrative data to identify population health trends, risk patterns, and care gaps., • Develop cohort analyses, risk stratification models, and longitudinal outcome tracking for chronic disease populations and CICIP high priority populations defined by the program measures., • Analyze utilization patterns, cost trends, and outcomes for value-based care initiatives., • Communicate any issues in a timely manner if they arise and present solutions to the problems. Epidemiologic Analysis • Apply epidemiologic methods such as:, • cohort studies, • cross-sectional analyses, • risk adjustment methods, • stratified outcome analysis, • multivariable regression modeling (e.g., linear, logistic, Poisson) to identify drivers and predictors of outcomes, • Use regression and predictive modeling to quantify the independent impact of clinical, operational, demographic, and social determinants of health variables on outcomes., • Assess population-level disease prevalence, incidence, and risk factors across patient populations., • Evaluate disparities in outcomes across demographic and social determinants of health variables., • Ensure that all analysis is actionable for business owners; enabling leaders of key areas to build, implement and identify solutions to address business performance gaps. Quality Measure Interpretation & Measurement • Interpret and operationalize quality measure specifications (e.g., HEDIS, CMS, Medicaid program measures)., • Translate measure specifications into analytic logic for data extraction and reporting., • Validate measure calculations and ensure alignment with program definitions., • Develop and maintain technical specifications and end-user documentation for team projects and analyses. Program Evaluation • Design and conduct program evaluations for population health interventions, CICIP, OIPP, and HAP initiatives., • Measure impact of initiatives on:, • clinical outcomes, • quality performance, • utilization, • patient experience, • CICIP program and process metrics, • Produce pre/post analyses and trend reports., • Review and evaluate program requirements to determine the value of participation and determine the need for the development of additional data extracts or reports to meet program goals. Reporting & Visualization • Develop dashboards, reports, and analytic summaries to support value-based care (VBC) program goals and performance monitoring., • Translate complex analytic findings into clear, actionable insights for clinical, operational, and executive stakeholders., • Apply data visualization and storytelling techniques to effectively communicate results and inform improvement strategies., • Collaborate with stakeholders to identify, prioritize, and refine business intelligence and reporting needs, establishing reporting standards and best practices., • Produce timely and accurate routine and ad hoc reports to support program monitoring and decision-making., • Support reporting and analytics requirements for grant-funded programs, Medicaid initiatives, and value-based contracts., • Identify opportunities to streamline reporting processes, improve data quality, and enhance report management efficiency., • Collaborate with University of Toledo Physicians (UTP) and University of Toledo Medical Center (UTMC) clinical, operational, and administrative stakeholders to support enterprise quality, safety, and performance initiatives for value-based programs., • Partner with informatics teams, EPIC analysts and data analysts to define data extraction logic as necessary., • Provide subject matter expertise on measure intent and program design., • Support clinicians and program leaders in interpreting analytic findings., • Leverage clinical, pharmacy, demographics and care management data to evaluate financial risk, identify medical economic trends and assess the performance of clinical programs for quality and financial outcomes. REQUIRED QUALIFICATIONS • Education: Bachelor’s degree in clinical analytics, public health, or health informatics with relevant work experience., • License and/or Certification: Epic Cogito, Clarity and/or Caboodle Certification and if not certified, willingness to get certified within first year of employment, • Skills:, • Expert proficiency with Microsoft Excel, Access, and other desktop analytics tools., • Intermediate proficiency with SQL, SPSS, SAS, or other data querying and statistical analysis tools., • Experience with business intelligence and data visualization tools such as Tableau or Power BI., • Experience with electronic medical records and healthcare data files., • Working knowledge of healthcare terminology including HCPCS, CPT, RBRVS, diagnosis and DRGs., • Ability to analyze complex datasets, interpret results, and present findings using appropriate analytic tools, charts, and visualizations., • Experience with epidemiologic study design and population-level analysis., • Experience conducting health outcomes analysis and applying program evaluation methodologies., • Ability to interpret healthcare quality measures and performance specifications., • Experience supporting population health initiatives and value-based care strategies., • Ability to communicate analytic findings clearly through effective data storytelling and visualization., • Ability to manage multiple assignments, adjust priorities based on organizational needs, and maintain confidentiality of sensitive information., • Ability to work independently while identifying and proposing solutions to business or program challenges., • Excellent written and verbal communication and organizational skills., • Highly self-motivated, organized, and able to establish priorities and coordinate work activities., • Strong analytical, quantitative, evaluative, and problem-solving skills., • Ability to operate effectively in an ambiguous or matrix organizational environment., • Years of Experience: 3+ years of experience in healthcare analytics, epidemiology, or population health. PREFERRED QUALIFICATIONS • Education: Master of Population Health (MPH) in Epidemiology or Biostatistics., • License and/or Certification: Epic Cogito, Clarity and/or Caboodle Certification, • Skills:, • Experience supporting value-based care programs or Medicaid initiatives., • Familiarity with risk adjustment methodologies (HCC, RAF scoring)., • Experience working with Epic., • Experience evaluating population health interventions or QI initiatives., • Knowledge of implementation of performance improvement or quality improvement frameworks., • Advanced understanding of healthcare terminology and operations including ICD, CPT, rev codes, PMPM, utilization rates, case mix, attribution methods, risk adjustment, quality metrics and episode groups., • Years of Experience: 5+ years of experience in healthcare setting WORKING CONDITIONS • Primarily sedentary in nature with occasional lifting of small objects weighing approximately 10 pounds; sitting; standing; walking; talking and hearing., • Primarily inside in well-lighted, ventilated areas., • Must be able to execute all terms and conditions set forth in the University of Toledo Physicians, LLC Employee Handbook, including, but not limited to:, • Works in a safety-conscious manner that supports and ensures safe work practices., • Complies with all UTP and other applicable UT policies and procedures and local, state, and federal regulations., • Interacts in a tactful, diplomatic, and humanistic manner with patients, care givers, and coworkers to provide a patient-centric environment.
