Position Summary The Licensed Vocational Nurse (LVN) coordinates and administers medical services to patients in a high-acuity Care Transition Center setting. This role includes assessing, screening, educating, and treating patients—many of whom are experiencing homelessness and co-occurring medical and behavioral health conditions. This position requires strong clinical judgment, the ability to respond to crisis situations, and a commitment to serving vulnerable populations. Essential Responsibilities • Perform LVN duties including patient intakes, rooming, medical histories, screenings, and recording vital signs, • Follow standardized clinic policies, procedures, and protocols, • Promote patient wellbeing through education and health promotion, • Coordinate care and schedule appointments (home health, specialists, PT/OT, dental, primary care), • Maintain accurate documentation for regulatory and funding compliance, • Conduct medication intake, verification, reconciliation, logging, audits, and patient education regarding prescriptions, • Support and direct Clinical Support Specialists (CSS) during shift, • Assist with training new hires, • Participate in peer review of patient medical charts, • Respond calmly and effectively to crisis or potentially hostile situations, • Work occasional overtime or alternate shifts as needed, • Perform additional duties as assigned Required Qualifications • Current California LVN license, • Two (2) or more years of clinical experience with a diverse adult population preferred, • Bilingual Spanish preferred, • Current CPR and First Aid certification, • Knowledge of LVN scope of practice, • Knowledge of medications, usage, interactions, and side effects, • Familiarity with community resources (Medicare, Medi-Cal, SSI, General Assistance), • Ability to observe, assess, and document patient conditions accurately, • Strong communication and patient education skills, • Ability to maintain professional relationships and detailed records, • Proficient computer skills, including EHR systems, • Ability to adhere to infection control and safety protocols, • Ability to support High Reliability standards Work Environment This position operates in an environment similar to a shelter setting serving individuals experiencing homelessness.
