Key Responsibilities • Conduct comprehensive physical therapy assessments to evaluate patients' mobility, strength, range of motion, and functional abilities, • Develop and implement individualized treatment plans tailored to each patient's specific rehabilitation goals and medical conditions, • Deliver evidence-based physical therapy interventions including therapeutic exercises, manual therapy, gait training, and mobility assistance, • Provide patient and family education on home exercise programs, proper body mechanics, fall prevention, and adaptive equipment use, • Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare team members to ensure coordinated care, • Maintain detailed, accurate documentation of patient progress, treatment plans, and outcomes in compliance with regulatory standards, • Monitor patient progress and adjust treatment protocols as needed to optimize rehabilitation outcomes, • Ensure patient safety and infection control practices during all home visitsRequired Qualifications, • Current, unrestricted Physical Therapist (PT) license in the State of Texas, • Doctor of Physical Therapy (DPT) degree from an accredited program, • Comprehensive knowledge of physical therapy principles, techniques, and best practices, • Excellent verbal and written communication skills with patients, families, and healthcare professionals, • Strong clinical reasoning and critical thinking abilities, • Ability to work independently with minimal supervision while managing a flexible schedule, • Compassionate, patient-centered approach with excellent interpersonal skills, • Valid driver's license and reliable transportation for home visits throughout the service area, • CPR certification (or willingness to obtain upon hire)Preferred Qualifications, • Previous home health physical therapy experience strongly preferred, • Experience working with geriatric populations and chronic conditions, • Familiarity with Medicare/Medicaid documentation requirements, • Competitive compensation ranging from $105,000 to $115,000 annually, • Flexible scheduling that promotes work-life balance, • Autonomy and independence in clinical decision-making, • Deep patient connections through one-on-one care in familiar home environments, • Professional growth opportunities with continuing education support, • Rewarding outcomes as you help patients regain independence and improve quality of life, • Supportive team environment with collaborative interdisciplinary care coordination
