The successful candidate will be expected to: • Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program)., • Support individuals with disabilities to utilize their services and funds to live in the community., • Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations., • Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence., • A Bachelor or Master of Social Work degree is preferred., • A college degree in Human Services or related field will be considered., • A high school diploma or equivalent is required., • 2 years of Case Management experience required. Higher education will be considered in lieu of experience., • Strong customer service and computer skills., • Ability to work in a fast-paced environment., • Knowledge of Microsoft Excel, Word, and Outlook., • Excellent verbal and written communication skills.
