We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. CAMBA operates 6 single adult shelters and 6 family shelters and one drop in center. Each shelter offers clients a supportive, structured therapeutic, safe, and drug-free facility. Homeless (often mentally-ill and substance-abusing) men and women are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing. CAMBA’s Flagstone Family Center provides comprehensive case management services to 160 families who are in a Tier II transitional residence program for homeless families. The facility consists of eight (8) four (4) story walk up apartment buildings on the east and west sides of the street. There will be two main entrances: one for the east side of the street and one for the west. Each building contains 20 apartments of varying sizes, each fully self-contained. Overview The Practice Facilitator is a healthcare professional essential for guiding, coaching, and supporting Value-Based Payment Programs within medical practices. This role manages relationships with practice leadership and drives transformation in quality improvement and patient-centered care models. The Practice Facilitator focuses on improving clinical outcomes, streamlining operational workflows, and enhancing the total cost of care for populations by promoting best practices and integrating supportive care delivery models. Frequent travel to healthcare practices and community sites is required to ensure direct collaboration and on-site implementation support. Hybrid schedules are based on program needs and leadership discretion. Salary Range: 50k-55K based on relevant experience and education. Major Duties: • Assess and monitor for safety and risk., • Establish immediate contact with families of the children who are placed into our care, and with their foster families., • Conduct required casework contacts with the child, parent, foster parent and collateral contacts (schools, service providers, medical personnel, mental health practitioners, and substance abuse counselors).
