Even small steps forward are acts of resilience.SUMMARYThe Critical Time Intervention Team Lead will manage and lead a multidisciplinary team delivering intensive, time-limited services to individuals experiencing mental health challenges, homelessness, with potential co-occurring substance use issues. The role involves coordinating the delivery of CTI services, ensuring high-quality care, and supporting the professional development of the team. The Team Lead will collaborate with community partners, stakeholders, and clients to help individuals transition smoothly from institutional to community-based care.RESPONSIBILITIES & DUTIES A representative summary of tasks to be performed is provided below. The duties and responsibilities of the position are to be carried out in a manner that is consistent with the mission, values, and operating principles of Jewish Family Services. • Through the state learning management system, state meetings as well as independent evidenced based research, the Team Lead will become subject matter expert on the Critical Time Intervention model., • Lead, supervise, and support a team of CTI care managers, care managers with lived experience, a nurse and a licensed clinician. Support staff with complex cases, crisis situations, and documentation requirements., • The Team Leader ensures fidelity to the CTI model consistently through their work in the program., • Provide ongoing training, coaching, and feedback to team members to ensure effective implementation of CTI principles., • Conduct weekly team meetings to monitor progress, share insights, and troubleshooting challenges., • Maintain relationships with hospitals, community-based organizations, social service agencies, and other relevant stakeholders to support client needs., • Oversee the case management and delivery of CTI services to clients, ensuring that services are provided in accordance with the CTI model., • Monitor and assess client progress throughout the critical transition period, ensuring that client needs are met within established deadlines., • Assume responsibility of information data management as required by OMH into the state utilized system., • Build and maintain relationships with community organizations to enhance client access to resources., • Ensure clients are connected to necessary services in the community, such as housing, healthcare, and employment., • Ensure management of the 24-hour referral hotline and ensure adherence to OMH guidance., • Ensure the team delivers culturally competent services, respecting the diverse backgrounds and needs of the clients., • Complete necessary documentation for legal, health, and social service purposes, ensuring compliance with privacy regulations (e.g., HIPAA)., • Promote a welcoming, inclusive, and non-judgmental environment for clients and for the team., • Provide continuous monitoring of JFS internal electronic health record (Medent) to ensure delivery of quality client services as well as monitoring of documentation and workflow., • Effectively utilize electronic systems including electronic documentation and reporting systems., • Provide education to the community and other JFS departments on Critical Time Intervention services and build strong referral network internally and externally., • Attend agency and department in-service training and staff meetings as well as any other agency related activities as required.QUALIFICATIONSEducation and Experience:, • Preference to Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor, (LMHC), or Licensed Marriage and Family Therapist (LMFT) who have the authority to act as an LPHA (Licensed Practitioner of the Healing Arts). Consideration given to Licensed Master Social Worker (LMSW)., • Must have active New York State licensure., • Minimum of 3-5 years of post-licensure experience in working with clients who have serious mental health illness, or co-occurring mental health and substance use disorders in a community-based setting., • At least one year of supervisory experience (or equivalent leadership experience overseeing staff and services)., • Experience with the CTI model or similar time-limited interventions is a plus. Knowledge, Skills & Abilities: • Leadership and team management skills., • Knowledge of mental health, substance use, housing, and social service systems., • Excellent communication, interpersonal, and crisis management skills., • Experience with case management and client-centered care models., • Ability to analyze and use data to improve program delivery., • Exhibited ability to effectively work within an inclusive and culturally and linguistically diverse environment., • Familiarity with case management software and documentation systems., • Strong internet research and computer skills. Required ability to utilize Microsoft Office tools effectively., • Preferred experience with external systems such as HealtheLink, PSYCKES etc., • Bilingual skills are desirable. WORKING CONDITIONS • Will work in the office and in the community; able to travel outside the office to various sites to attend meetings and provide support services. Notably the CTI program covers 5 Western New York counties to which travel will be required (Erie, Genesee, Niagara, Orleans & Wyoming)., • Must have access to a reliable vehicle, possess a valid, clean driver’s license and be sufficiently self-insured with liability insurance in the amount of $100/$300k., • Flexible hours including days and some evenings and/or weekends. PHYSICAL REQUIREMENTS • Physical activities and efforts required working in an office environment., • Visual acuity is sufficient to maintain system of files and reports containing computer-generated and handwritten documents., • Auditory acuity sufficient to communicate with staff, clients, and others by phone and in person., • Mobility is sufficient to conduct regular duties within a normal office environment and community. COMPENSATION & BENEFITS • Competitive salary of $66,500 - $72,000 annually, commensurate with experience & qualifications., • Health, Dental, and Vision insurance., • Accrued Paid Time Off (PTO) of 4+ weeks in addition to a paid agency wide shut down between Christmas and New Year's Day., • 401k retirement plan with agency contribution of 4%., • 13+ observed holidays annually., • Reduced full-time work week of 35 hours and early close on Fridays.
