Job DescriptionAbout the Role The Patient Care Manager will lead care coordination efforts, support patient engagement, and ensure the delivery of high-quality, patient-centered services across virtual care platforms. This role serves as a key liaison between patients, healthcare providers, and internal teams, ensuring seamless communication and effective care management.
