Job Description Program Population: The Housing for Health Recuperative Care Center, located on the LAC Outpatient Center campus, is seeking qualified RNP and PA’s with a keen sense of empathy, enthusiasm, and passion for caring for the homeless population. The RNP/PA will be offering routine medical care in an outpatient setting for the residents of the Recuperative Care Center. The facility will provide interim housing to individuals who would benefit from this resource, such as those being discharged from a hospital admission, ED, or clinic setting, and are especially vulnerable to further decompensation or relapse of their medical conditions. Key Responsibilities: • Provide comprehensive primary and preventive medical care to underserved patient populations, including individuals experiencing homelessness., • Conduct initial intake evaluations and ongoing assessments; diagnose, treat, monitor, and manage acute and chronic medical conditions, including newly identified health concerns., • Continue and coordinate treatment plans initiated by hospitals, emergency departments, and primary care providers to ensure continuity of care., • Manage an assigned patient panel, contributing to population health initiatives and quality outcomes, including HEDIS measures., • Collaborate with primary care providers, specialists, and interdisciplinary teams to deliver coordinated, patient-centered care and achieve optimal health outcomes., • Participate in care conferences and support discharge planning, including identifying barriers to housing, establishing care goals, and facilitating appropriate transitions of care., • Provide patient education using appropriate tools and methods, including motivational interviewing, to support engagement, self-management, and adherence to treatment plans., • Coordinate and review referrals, including those from Housing for Health access and referral teams, ensuring appropriate follow-up and continuity of services., • Provide consultation and clinical guidance to nurses and other healthcare staff regarding patient care., • Collaborate with leadership, including the Medical Director, on clinical oversight, performance feedback, and support of midlevel providers and clinical staff., • Assist in planning, directing, and improving primary care team activities within the scope of the physician’s specialty., • Participate in quality improvement initiatives, including utilization review, peer review, and quality assurance activities., • Support compliance with regulatory requirements, including execution of Delegation of Services Agreements for midlevel providers in accordance with standardized procedures and protocols., • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors., • Participate in required safety and compliance trainings and apply learned practices in daily work activities., • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace., • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards., • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role., • Participate in JWCH’s customer service training called AIDET on an annual basis., • Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions., • b. Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes., • Current and unrestricted California Physician & Surgeon License., • Current and unrestricted DEA Certificate., • Current BLS/CPR Certification (American Heart Association or American Red Cross)., • Minimum of one-year experience working with adults who are homeless, • Knowledge of and practice in the methods of harm reduction, • Experience working directly with individuals with both moderate to severe mental health and substance use diagnoses Note: If you become a JWCH provider, you may be able to participate in Loan Repayment Program. A majority of our clinics are NHSC approved sites. The National Health Service Corp may be accepting applications Loan Repayment Program awards. The program is open to licensed primary care medical, dental, and mental and behavior health providers who are employed or have accepted an offer of employment at an NHSC-approved site. Awardees receive up to $50,000 in exchange for two years of service if they practice in a HPSA of 14 above (Tier I). Awardees can also receive up to $30,0000 in exchange for two years of service if they practice in a HPSA of 13 and below (Tier II).
