Jefferson Center is seeking a Community Outreach Liaison to join our recovery-oriented, Mobile MAT team. Our model helps people thrive by continuously revolutionizing healthcare delivery through a data-driven, team-based approach that integrates behavioral health, substance use, and basic needs coordination. Mobile MAT is a recovery team, working in rural areas where clients have challenges around SUD and mental health services. Mobile MAT uses data and firsthand encounters to bring solution-based treatment to this population. Our team members travel to Gilpin, Clear Creek and rural Jefferson County, assessing and providing treatment. As a Community Outreach Liaison, you will play a vital role in connecting individuals to resources, fostering recovery, and addressing barriers to care. This position also supports community partnership to diversify resources/referrals, expose need, provide education and uphold a continuum of care for this population. You may be a good fit for our team if your values align with ours: * • The people we serve are supported around MAT services and other harm reduction interventions., • The people we serve often face complex challenges that are best supported through a team-based approach., • Bachelor’s degree in psychology, social work, or related field required., • One or more years of experience in a behavioral health or social services setting preferred., • Experience with Stages of Change and Harm Reduction models preferred., • Strong communication, organizational, and computer skills required., • Ability to multi-task, prioritize, and manage time independently., • Demonstrated ability to work effectively with diverse populations and to understand the relationship between social indicators (e.g., racism, access barriers, poverty) and health outcomes. Care Coordination & Basic Need Management • Engage clients in trauma-informed treatment process and goal setting., • Provide care coordination, basic needs management, and linkage to medical, social, educational, and housing resources., • Administer evidence-based measurement tools regularly and use outcome data to drive care planning and collaboration., • Connect with the Mobile MAT therapist to assess risk and lethality assessments and coordinate with emergency services as necessary., • Support clients with accessing basic needs and promote protective factors that support recovery and stability., • Utilize data and new operational practices to adapt caseload complexity and improve outcomes., • Conduct outreach on and around community partners to increase awareness of Mobile MAT services., • Provide field-based assessments, referrals, and linkage to Jefferson Center and community services, including shelter, housing, medical, identification, and employment supports., • Maintain accurate documentation and demographic data using Mobile MAT tracking systems and Jefferson Center record keeping standards., • Serve as a knowledgeable liaison to community partners, and service providers to coordinate holistic, recovery-oriented care., • Exhibit enthusiasm, courtesy, adaptability, flexibility, and a spirit of cooperation in the work environment., • Maintain effective interpersonal relationships with clients, peers, supervisors, and community partners., • Attend team meetings, clinical supervision, and trainings as required., • Submit all required documentation within established timelines., • Maintain a positive focus and customer service orientation toward clients, staff, and the community., • Participate in supervision and problem-solve collaboratively with leadership., • Attend mandatory trainings and adhere to individualized training plans. Salary Grade 50 (Bachelor’s Degree) - $55,000 ($26.45/hr)* Additional Salary Information*: • The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week.
