Job Description The primary role of an early intervention physical therapist is to use knowledge and skills specifically related to motor function, self-care function, assistive technology and medical/healthcare science to provide unique care to children and their families regardless of disease or condition. Responsible for providing evaluations, assessments and ongoing physical therapy for children age birth to three years of age as well as participation in case management. New Heights of Northeast Florida provides home and clinic-based therapy to children in need of early intervention. As a therapist at New Heights, you can expect to: • Work within a coaching model to assist families with their child’s development and ability to participate in meaningful activities., • Find resources in the community that are suited to meet the family’s needs, participate in transition to preschool or other community program, • Help families make decisions on adaptive equipment when and if needed by the child. Facilitate and complete the assistive technology process from evaluation to equipment maintenance., • Evaluate children using standard tools to measure attention, cognition, nerve integrity, body mechanics, balance, mobility and other physical limitations., • Prepare written documentation and interpretation of evaluation results and assessments communicate with physicians, families, caregivers and other team members., • Communicate with families and caregivers on how to incorporate strategies into their daily routines., • Develop a plan of care for each child including appropriate strategies and techniques that will guide intervention., • Provide instruction to child and family related to those strategies. Provide education to families on child development and progression as well as early intervention information., • Work in collaboration with Northeastern Early Steps for a high quality, smooth continuum of care for each family and child., • Participate in an Early Steps Cohort training program within the first year., • Belief in, and Commitment to, New Heights’ mission to “turn disabilities into capabilities”, • Passionate for those that we serve, • Willingness to go Above and Beyond, • Provide Exceptional Customer Service
