Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. About the Role The Case Manager work directly on the reunification process of the child along other stakeholders as per ORR Policy and Regulations. The Case Manager ensure all the child needs (medical, legal, educational, and emotional,) are meet while in care. The caseload assigned as per ORR regulations is up to 8 cases per case manager. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. Job Type: Full-time, Part-time, or Per Diem Pay: $22.18/hour Responsibilities include: • Develop and implement lesson plans for identified groups of people supported based on individual program plans., • Perform, document, and compile results from assessments to determine interests, skills, abilities, learning and training needs of people supported., • Match individuals with training, volunteer opportunities, internships and employment related to their desired outcomes and assessment results., • Lead the teaching of people receiving supports functional skills as determined by each individual’s program plans and assessments. This may include but is not limited to work readiness and/or pre-employment skills, behavior management, independent living, community integration, and/or other skill development., • Prepare and organize materials for instruction and lead/model hands-on instructional activities in site-based and community-based settings based on assignment and/or location., • Attend to personal hygiene needs of program participants, which may include toileting, changing, feeding program participants, and showering as the situation warrants., • In a timely manner, complete, organize, and maintain electronic/manual documentation and program materials for people supported according to guidelines as set forth. This includes: valued outcome and behavioral data collection, monthly notes, habilitation plans, behavior plans, regulatory information, program books, replenishing materials needed for activities, and maintaining an organized work/program area based on assignment., • Meet requirement to become and maintain certification as a Registered Behavior Technician based on assignment. This may include providing a variety of training, intervention, and teaching procedures., • Operate Agency vehicles for transportation and travel training of the people receiving supports as assigned., • Administer prescribed medications to people receiving supports after successful completion of AMAP medication administration course based on assignment and/or location., • Participate in planning and team meetings; act as liaison with other community organizations, volunteer sites, and agencies., • Perform required bus duty functions as required. Duties included traffic monitoring, paging, loading, and unloading, escorting and attendance documentation., • Write case notes, reports incidents, and provides information on incidents as required., • Provide oversight and guidance to staff members, demonstrating job duties and reviewing/completing department specific orientation. May be assigned to mentor new staff members., • Responsible for health, welfare, and safety of program participants both on program site and while in the community with program participants.
