Job Description Position Summary: The Department of Behavioral Health and Intellectual disAbility Services (DBHIDS) and the Philadelphia Fire Department (PFD) Emergency Medical Services Division have established an Alternative Response Unit (ARU) to work in the epicenter of the Opioid epidemic. The ARU will consist of a Coordinator, Paramedic Lieutenant, and a Case Manager, working primarily within the PFD response system. The team will respond to persons who are treated for opioid overdoses who may refuse treatment, family members or any individual who are interested in treatment and require assistance with linkage. The ARU Case Manager will have the knowledge, resources and information of available treatment modalities and connections to social services. This position will support individuals on their road to recovery. The ARU Case Manger will be supervised by the ARU Coordinator and work in conjunction with the team to engage, assess, link and conduct follow-up. Should the individual refuse transfer to an emergency department, the ARU Case Manager and team will engage with the individual and provide information related to accessing drug treatment, harm reduction services (i.e., syringe exchange), overdose education and naloxone distribution. If the individual is amenable to services, the social worker will collect contact information to ensure linkage. When not responding to a call, the ARU staff will be following up with people who were previously resuscitated and refused transport. The case manager, in conjunction with the PFD will be responsible for engaging with community organizations and members. The case manager will also provide overdose education and training to individuals and families. Duties and Responsibilities: Provide outreach and engagement to the individuals in the epicenter of the opioid epidemic. Works on-site collaboratively with the PFD-EMS services when dispatched for a possible overdose. Attends periodic staff meetings and serves as a representative on Opioid-related work groups; confers with coordinator on difficult problems; prepares reports and correspondence; keeps records of all assigned cases, completes documentation for follow up and data collection. Job Description Position Summary: The Behavioral Health and Justice Division (BHJD) is committed to working towards reducing criminal justice involvement for Philadelphians with behavioral health challenges, including Serious Mental Illness (SMI). BHJD organizes its work according to the Sequential Intercept Model, identifying opportunities to intervene at various points of criminal justice involvement to deflect and divert individuals away from further penetration of the system and into the behavioral health system. By helping connect individuals to needed treatment and support services that promote stability in the community, BHJD believes that Philadelphians are less likely to be arrested and can achieve health, well-being, and self-determination. BHJD also aligns with Stepping Up and focuses its efforts on lowering the number of Philadelphians with SMI admitted to the Philadelphia Department of Prisons (PDP), reducing their length of stay, connecting them to treatment/supports upon release, and reducing recidivism. The Forensic Support Team (FST) of BHJD was established to meet the unique needs of Philadelphians who are either: 1. Adjudicated as not competent to stand trial on only misdemeanor charges and committed to a State hospital (e.g., Norristown State Hospital) or 2. Adjudicated as non-restorable. The FST works within PDP and Norristown State Hospital (NSH) in order to assist individuals while in jail, when transitioning from jail to NSH, and when reentering the community typically under the court’s jurisdiction. To support this forensic population, the FST helps individuals navigate the Philadelphia judicial and behavioral health systems while also seeking to address each individual’s unique legal, behavioral, medical, physical, cultural, and/or financial needs. The Clinical Assessor becomes an important part of a team comprised of: one (1) FST Supervisor (a licensed psychologist); one (1) FST Coordinator, three (3) Clinical Assessors, and one (1) Post-Doctoral Psychology Resident. The Clinical Assessor will complete clinical assessments and provide system navigation for justice-involved individuals with behavioral health challenges who fall into either of the aforementioned priority groups. In this capacity, the Clinical Assessor is responsible for determining the appropriate level of care (LOC) for the individual via a procedure that is consistent with a strengths-based, biopsychosocial recovery model. The procedure relies on State hospitals and jails to provide access to the individual as well as clear documentation and consultation about the individual so an informed decision can be made about the appropriate LOC and the individual’s risk for violence. The decision about the LOC is based on the individual’s: aspirations, goals for recovery, strengths, support system, risk for future violence, psychiatric diagnosis, medical diagnosis, psychiatric history, cognitive or physical limitations, results from any psychological measures or medical tests, current medications, incident reports, participation in recovery services, current treatment team input, and privileges earned. With this information and documentation, the Clinician Assessor collaborates with the Philadelphian and their treatment team in an effort to agree on an appropriate LOC as well as a re-entry plan and, when necessary, present the plan to the appropriate criminal court. Duties and Responsibilities: The Clinical Assessor helps individuals with SMI "navigate" the maze of treatment programs, the court system, and support services upon reentry into the community. The Clinical Assessor reduces barriers that keep individuals with SMI from accessing timely treatment by identifying behavioral health needs and directing individuals to appropriate resources. An effective Clinical Assessor is a: Relationship builder Building effective working relationships is the foundation of effective forensic navigation. To improve coordination of care for people with mental illness and to reduce barriers to reentry, navigators must work with different groups of people: individuals who are incarcerated, healthcare teams within the PDP, criminal justice partners, families, NSH staff, and resource providers within the community. Assessments, referrals, legal statuses, and treatment histories are just a few things the Clinical Assessor needs to track and, by law, this information must be kept confidential and secure. It is important that the Clinical Assessor stays organized and uses a systematic approach to information management. Therapy is focused on resiliency factors and connection to community-based supports as identified by the treatment team. Mobile therapy services consist of individual therapy, family therapy, development and revision of the ITP (as needed), assistance with crisis stabilization and assistance with addressing problems the child, youth or young adult has encountered. The BSC also coordinates and consults with other service providers regarding behavior plans and interventions and discussion with the child’s, youth’s or young adult’s treatment team about the behavioral interventions that should be used and when they should be used. BSC services can be used to address behaviors in the school, home, or any other place where a child, youth or young adult needs behavior support (i.e.- across settings) and to learn strategies to better succeed. For non ASD FBA’s – the FBA certification is sufficient., • Supervision Requirements:, • BSC clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides BSC services supervises BHT treatment team members, the BSC must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: BSC clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • BSC's must complete at least 40 hours of training within the first year and 20 hours each year thereafter. These training hours must be Department-approved and related to the specific job functions and in accordance with their individual training plan. The training hours will also include mandatory agency and/or funder or state requirements., • Valid Act 34, Act 151 and Act 114 clearances. Work location is subject to change based on business needs. On-Call: Participation in the on-call rotation is a requirement of this position. It is anticipated that an incumbent in the position would be on-call approx. once every eight (8) weeks for two (2) weekdays per month and one (1) weekend. Job Description: A Registered Nurse for Home Infusion inserts, monitors and removes an IV used to give medications, blood products and nutrition to a patient. During the IV therapy, the nurse monitors the patient for adverse reactions and ensures that the tubing, bandages and needle stay in place. When a patient needs blood levels checked, an IV nurse performs a veinous phlebotomy. During the entire IV therapy process, the nurse obtains sets of vital signs, completes the physical assessment and review of systems, documents all patient-related activities and records the medications they use. Provides and coordinates all aspects of staffing services to the patient with the assistance of the Company Directors, Nursing Coordinator and the Director of Nursing. Communicates with the patients to determine problems and/or solutions regarding their treatment. The Per Diem RN also conveys collected documentation from patient to all necessary internal and third-party external staff regarding scheduling and other pertinent patient information. Performs a variety of duties both clinical and nursing notes documentation. This position requires skill in planning, organizing, scheduling and coordinating the delivery of patient care by all staff personnel including but not limited to complete documentation of nursing care and submission of nursing notes. Job Description PROGRAM SUMMARY To promote community safety through services that reduce drug abuse and violence, as well as to help at-risk youth develop positive alternatives that will return them to the mainstream of society. Since its inception in 1989, PAAN has utilized its expertise to meet these objectives through street-based community outreach and intervention programs. Promote positive transformation through community outreach and creating programs that prevent juvenile delinquency and adult criminality. We focus on job readiness, job training and mentoring. The Community Crisis Intervention Team concept is based on the work of PAAN’s predecessor, Philadelphia’s Crisis Intervention Network. This nationally-recognized agency, founded in the mid-1970s by James Mills and Bennie Swans, effectively addressed gang violence, drug abuse and drug trafficking and was credited with dramatically decreasing gang-related deaths in Philadelphia for over a decade. The Community Crisis Intervention Program (CCIP) aims to replicate Crisis Intervention Network’s approach to combat today’s community violence. The Model will use credible messengers who are indigenous to Philadelphia’s most violent neighborhoods as outreach workers with the mission of fostering meaningful relationships in these communities, steering those involved in criminal activities into alternative positive choices, mediating neighborhood conflicts, and responding to neighborhood crises. The AV Intervention Case Manager will focus primary of supporting the following AV program: The Intervention Case Manager (ICM) is responsible for working with clients ages 14-18 who are referred and identified primarily through PDAO. This position requires experience working with high-risk individuals from diverse backgrounds. DIRECT REPORT: This position will report to PAAN’s Director of Human Services. PRIMARY RESPONSIBILITIES: • Engage and assist participants who have criminal justice system involvement to facilitate linkage to resources, programs, employment, benefits, etc., and support/ promote a healthy and safer lifestyle., • Manage a caseload of 30-35 at risk individuals ages 16-34, and meet with and/or contact these participants a minimum of once per week to review progress and ensure the participant is progressing, and refraining from any criminal activity., • Make entries into participants’ weekly contact system. Creating a narrative through case notes for each participant on the case load. These weekly contacts are to create a rapport with the participant that will aide in the overall progress of each participant. Each weekly entry will be accurate case notes and documentation related to the program., • Produce accurate and comprehensive documentation of information regarding all participants’ interactions, caseload progress, inter or intra-agency contacts or general information pertinent to a client’s progress. Create weekly plans of action for struggling youth, manage the progressive discipline of each youth on your caseload., • Maintain confidentiality obtained through job duties regarding participants’, and outside agencies so that sensitive information is only given on a "need to know" basis, and ensure files are secure to protect the participants’ confidentiality., • Conducts risk/need assessments and creates individual goal plans for each assigned participant. Meet weekly with the Human Services Coordinator (HSC), to create this plan, and reassess what is needed for the participant on a constant basis., • Make weekly/monthly visits to community participants and partnerships. This position will be an active member of the case manager process, and making homes visits is expected to happen on a regular basis., • Other duties as assigned by HSC or HSD. EDUCATION AND QUALIFICATIONS: • A high school diploma is required, a bachelor’s degree in human services, criminal justice, social work, education, or other related fields preferred., • Minimum of 1-2 years’ experience in case management, or social work., • A passion for violence prevention and for serving Philadelphia’s most at-risk neighborhoods., • Must possess excellent leadership, problem solving and analytical thinking skills., • Must have strong interpersonal and teamwork skills, and can work independently with minimum supervision;, • Must possess excellent relationship building skills, • Must be detail-oriented, and possess excellent writing, speaking, facilitation, coaching and computer skills;, • Must obtain a Criminal Background, Childline, and FBI Clearances;, • Must be proficient in Microsoft Office products (Excel, Word, etc.) and database experience., • Bilingual Spanish/English strongly preferred UAC/PAAN is an Equal Opportunity Employer

Job Description General Functions: The employee in this classification shall report to the Services Coordinator/Program Director. The employee is part of the treatment team. The employee performs a treatment function emphasizing accountability established through the funding source and according to agency policy.
