Job Description Purpose of the Job: The Family Care Manager serves as the primary point of contact for families who are enrolled in the Haven Program and have complex nees including, but not limited to, housing and financial instability, unmet health and mental health needs and family stressors. Essential Functions: (All functions are essential unless noted otherwise) • Comprehensive Family EngagementBuild strong, trusting relationships with families presenting with intensive or complex needs, serving as a single, consistent point of contact to reduce duplication of services, • Hotline Assessment & PlanningConduct thorough intake and ongoing assessments to identify family strengths, needs, and barriers. Develop integrated service plans that streamline supports and reduce involvement from multiple programs., • Service Coodination & NavigationAct as the lead coordinator to connect families with essential resources (housing, healhtcare, education, financial support, legal services, etc.) while consolidating services across agencies and minimizing fragmentation., • Advocacy Across SystemsRepresent family needs within child welfare, criminal justice, healthcare, school, and social service systems to ensure services are unified, family-centered , and non-duplicative., • Crisis Response & StabilizationProvice timely intervention in urgent situations, help families de-escalate crises, and work toward long-term stability through consistent follow-up and coordinated care., • Skill-Building & Family EmpowermentSupport caregivers in strengthening parenting, communication and coping skills, with the goal of increasing family resilience and decreasing reliance on multiple outside programs., • Cross-Agency CollaborationWork closely with community partners to align services, share information (as permitted), and coordinate efforts to families experience a seamless system of care., • Documentation & AccountabilityMaintain detailed, timely case records that track progress, service reduction, and family outcomes, ensuring compliance with program and funding requirements. Minimum of 2-3 years of experience working directly with children and families, particularly those impacted by trauma, poverty, domestic violence, child welfare, or other complex needs. Experience in case management, advocacy, counseling, or family support services strongly preferred. Bachelor's degree in social work, human services, psychology, or a related field preferred but not required.
