Job Description The incumbent is responsible for providing direct patient care and clinical support in accordance with established policies, procedures, and standards of care. This role supports nursing staff and ensures a safe, compassionate, and efficient patient experience. • Administer assigned patient care tasks within scope and training., • Conduct initial pain screening by asking patients standardized pain assessment questions (Yes/No); escalate findings requiring further assessment to the Registered Nurse (RN)., • Perform Patient Care Technician (PCT) functions, including but not limited to:, • Vital signs (V/S), • Intake and output (I&O) monitoring, • Daily weights, • Patient ambulation and mobility assistance, • Respond promptly to patient call lights, bells, intercom systems, and other alert mechanisms to assess and address patient needs., • Turn, reposition, and assist bedridden patients to prevent discomfort and skin breakdown., • Provide physical assistance with activities of daily living (ADLs), including getting out of bed, bathing, dressing, toileting, standing, walking, and basic exercises., • Review and adhere to patients' dietary restrictions, food allergies, and preferences to ensure appropriate meal delivery., • Measure and accurately document food and fluid intake as well as urinary and bowel output; report notable changes to nursing or medical staff., • Obtain and record vital signs such as temperature, blood pressure, pulse, and respiratory rate as directed., • Gather and communicate relevant patient information from caregivers, nurses, and physicians regarding patient condition, treatment plans, and appropriate activities., • Observe patients for signs or symptoms requiring medical attention, including bruising, open wounds, cognitive changes, abnormal vital signs, signs of distress, or blood in urine., • Document and report patient observations, behaviors, complaints, and physical symptoms to nursing staff in a timely and accurate manner., • Remind patients to take prescribed medications or nutritional supplements, as directed., • Ambulatory Care, • Workflow Implementation, • High school diploma or GED (must be completed and in hand), • Minimum of two (2) years of Obstetrics (OB) experience in a hospital or clinical setting, • Current Basic Life Support (BLS) certification from the American Heart Association, • Active California Licensed Vocational Nurse (LVN) license, • Medical, dental & vision, • Critical Illness, Accident, and Hospital, • 401(k) Retirement Plan - Pre-tax and Roth post-tax contributions available, • Life Insurance (Voluntary Life & AD&D for the employee and dependents), • Short and long-term disability, • Health Spending Account (HSA), • Transportation benefits, • Employee Assistance Program We're partners in transformation.
