Job Description Case Manager Job Description The role of the case manager is central to the day-to-day functioning of any residential program. This individual assumes primary responsibility for establishing the house routing; for orchestrating all house activities; for ensuring that the house itself is kept clean and in good repair; for ensuring that each resident attends all scheduled programs and appointments; for ensuring that all medications are ordered and refilled as necessary and for establishing the tone of the house. The following tasks represent some of the daily expectations of the case manager: 1. Counting all controlled medications ensuring each resident has enough and there are none misplaced/missing; Any medication sheet which shows a five day supply or less should initiate re-order or the doctor should be asked to “call in” a new prescription if no additional refills are available.; making sure all residents are taking prescriptions as prescribed; checking to make sure all residents are prepared to attend regularly scheduled activities as well as medical or clinic appointments; make sure all medications and dosages for each resident are up to date in system., 2. Checking the whereabouts of all residents so that each is "present or accounted for"; checking the overall condition of the houses., 3. Checking to see if each resident has properly performed personal hygiene activities (showering, weather appropriate clothing, dressed in clean and neat clothing); checking cleanliness of all houses (kitchen, bathrooms, living space, bedrooms); assist residents in cleaning as needed;, 4. Using restorative services as a guideline to make sure that program activities are scheduled throughout the day and evening, which will allow every resident the opportunity to gain the skills necessary in order to move to the next level of independent functioning; make sure staff are assigned to carry out these activities and that residents are scheduled to involve themselves in them., 5. Document all conversations regarding each resident; make sure that all appointments, which are received verbally, are noted in the log book as well as a calendar for the day of appointment; make sure that all doctors orders for meds are faxed directly to the appropriate pharmacy; make sure contact info for all residents doctors, treatment teams, other providers are all up to date in system., 6. Provide 1-2 shopping trips a week for residents to buy their own household supplies and food items, 7. Plan, in concert with supervisor and other staff, for the conduct of evening and weekend activities such as shopping, field trips or purely recreational events; make sure funds are available to do such activities., 8. All rooms should be inspected for cleanliness and neatness; all rooms should be inspected to ensure that each contains all necessary items for daily living (sheets, towels, blankets, personal grooming, supplies), 9. Participants are provided with a variety of services to promote housing stability and individual well-being. Housing Retention Case Manger is responsible for following up with Coordinated Entry referrals, for completing the lease packets, ensure the administrative compliance and collection of rent, delivery of rent, for site owned or managed by Westhab. The Housing Retention Case Manager must diligently work to protect and preserve the quality of life in, and the value of, the properties which they are assigned to manage. This position requires a 24/7 work phone availability in case of any emergency. DUTIES & RESPONSIBILITIES: • Responsible for conducting clients’ intakes and obtain needed documentation to ensure clients eligibility for the referred department/program. Maintains monthly and accurate and up today progress notes in HMIS as directed., • Able to travel within the Westchester County and attend DSS/SSI/SSD or medical appointment for the clients if needed., • Able to work one or two weekend day in the year as requested by Director, if needed. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. About the Role The Case Manager work directly on the reunification process of the child along other stakeholders as per ORR Policy and Regulations. The Case Manager ensure all the child needs (medical, legal, educational, and emotional,) are meet while in care. The caseload assigned as per ORR regulations is up to 8 cases per case manager. What You’ll Do · Case management responsibilities include all necessary tasks as required by the Federal Office of Refugee Resettlement. · Works closely with Unification Specialist (TPG) to ensure timely and safe reunification process for all minors in care abiding by ORR Field Guidance 24. · Develops and maintains contact with youth and their service providers for medical, social, educational, and other related service needs. · Follow up with legal service providers regarding the timely submission of any documentation or correspondence needed for the legal relief process. · Maintains professional and timely communication with stakeholders including , GDIT, Legal Service Provider, The Young Center, ICE regarding the child case. · Performs discharge and release functions to ensure that each youth has a safe, appropriate and prompt release plan. · Documents weekly client contacts and maintain case files up to date and audit ready in accordance with regulations.
